MC4R-rs6567160位点基因多态性与非酒精性脂肪性肝病患者纤维化程度的相关性OA
The correlation between the polymorphism of MC4R-rs6567160 locus gene and the degree of fibrosis in non-alcoholic fatty liver patients
目的:探讨MC4R-rs6567160位点基因多态性与非酒精性脂肪性肝病(NAFLD)间的相关性.方法:选取NAFLD 患者2 032 例作为研究对象(NAFLD 组),分为无纤维化、轻度及中重度纤维化3 个亚组;选取同期健康管理体检中心体检的健康人1 920 例为对照组.收集所有受试者临床指标及实验室指标,同时进行基因分析,比较组间受检者实验室指标及基因型的差异,分析 MC4R 基因多态性与 NAFLD 患者肝纤维化程度的相关性.结果:CC 基因型NAFLD 组患者显著高于对照组(P<0.05).肝纤维化患者 CC 基因型肝功能指标、肝纤维化及肝硬度指标均显著高于TC 型和 TT 型,且 TC 型明显高于 TT 型(P<0.05).肝组织纤维化程度越严重则 CC 型基因占比越高,肝功能指标、肝纤维化及肝硬度指标越差(P<0.05).ALT、AST、GGT、HA、PCⅢ、Ⅳ-C、LN、LSM、CAP、APRI、FIB-4 及 MC4R-rs6567160基因型(CC 基因型)均是影响 NAFLD 患者肝纤维化程度的独立危险因素(均 P<0.05).MC4R 基因的 rs6567160 位点(CC 基因型)在预测 NAFLD 患者纤维化进展中,AUC 值分别为0.862 和0.963(P<0.01).结论:MC4R-rs6567160 位点基因多态性与 NAFLD 密切相关,而携带 CC 基因型患者可能肝纤维化的风险较高.
Objective:To investigate the association between MC4R-rs6567160 polymorphism and non-alcoholic fatty liver disease(NAFLD).Methods:A total of 2,032 NAFLD patients admitted to Chengdu Fifth People's Hospital were enrolled and stratified into no,mild,and moderate-to-severe fibrosis subgroups,alongside 1,920 healthy controls.Clinical and laboratory parameters were collected,and genotyping was performed via PCR.Group differences were compared to analyze the correlation between MC4R polymorphisms and the severity of liver fibrosis in NAFLD.Results:The frequency of the CC genotype was significantly higher in the NAFLD group compared to the control group(P<0.05).Patients with the CC genotype exhibited significantly higher levels of liver function markers,fibrosis indices,and liver stiffness measurements compared to those with TC and TT genotypes(P<0.05),with TC being intermediate.The proportion of the CC genotype increased with the severity of liver fibrosis,and corresponding clinical markers deteriorated accordingly(P<0.05).Alanine aminotransferase,aspartate aminotransferase,gamma-glutamyl transferase,hyaluronic acid,procollagen type Ⅲ,type Ⅳ collagen,laminin,liver stiffness measurement,controlled attenuation parameter,aspartate aminotransferase to platelet ratio index,fibrosis-4 index,and MC4R-rs6567160(CC genotype)were identified as independent risk factors for liver fibrosis severity in NAFLD patients(all P<0.05).ROC analysis demonstrated that the CC genotype had AUC values of 0.862 and 0.963(both P<0.001)in predicting the progression from no fibrosis to mild fibrosis and from mild to advanced fibrosis,respectively.Conclusion:The MC4R-rs6567160 polymorphism is significantly associated with NAFLD,and patients carrying the CC genotype are at a higher risk of liver fibrosis.These findings highlight the potential of the CC genotype as a biomarker for early diagnosis and risk stratification in NAFLD management.
