脊柱骨折患者术后压力性损伤风险预测模型的构建OA
Construction of a Prediction Model for Postoperative Pressure Injuries in Patients with Spinal Fractures
目的:探讨脊柱骨折患者术后压力性损伤风险预测模型的构建.方法:回顾性分析 2022 年 4 月—2025 年 3 月蚌埠医科大学第一附属医院收治的 168 例脊柱骨折患者的临床资料,根据是否发生压力性损伤分为压力性损伤组(n=34)及非压力性损伤组(n=134),收集患者的一般资料,多因素 logistic 回归分析,并建立风险预测模型、绘制列线图进行评估.结果:单因素分析结果显示,两组肥胖、潮湿程度、活动能力、营养摄取能力、摩擦力及剪切力比较,差异有统计学意义(P<0.05);logistic 结果显示,肥胖、潮湿程度、活动能力、营养摄取能力、摩擦力和剪切力是压力性损伤发生的影响因素.ROC 曲线下面积为 0.885,表明该模型具有较好的预测能力,DCA 曲线显示,在较宽的阈值概率范围内,该模型的临床应用可提供净获益,校准曲线显示预测概率与实际概率接近,拟合良好.结论:研究构建的预测模型能够更好地预测脊柱骨折患者术后压力性损伤发生情况,为临床采取有效措施提供参考.
Objective:To explore the construction of a prediction model for postoperative pressure injuries in patients with spinal fractures.Method:A retrospective analysis was conducted on the data of 168 patients with spinal fractures admitted to The First Affiliated Hospital of Bengbu Medical College from April 2022 to March 2025.The patients were divided into the pressure injury group(n=34)and the non-pressure injury group(n=134)based on whether they developed pressure injuries.General patient information was collected,and a multivariate logistic regression analysis was performed to establish a risk prediction model and draw a nomogram for evaluation.Result:Univariate analysis showed that there were statistically significant differences in obesity,moisture level,activity ability,nutritional intake ability,friction force,and shear force between the two groups(P<0.05).Logistic regression analysis indicated that obesity,moisture level,activity ability,nutritional intake ability,friction force,and shear force were influencing factors for the occurrence of pressure injuries.The area under the ROC curve was 0.885,indicating that the model had good predictive ability.The DCA curve showed that within a wide range of threshold probabilities,the clinical application of this model could provide net benefits.The calibration curve demonstrated that the predicted probability was close to the actual probability,indicating a good fit.Conclusion:The constructed prediction model can better predict the occurrence of postoperative pressure injuries in patients with spinal fractures,providing reference for clinical measures.
