基于双向两样本孟德尔随机化的情绪波动与非酒精性脂肪性肝病因果关系分析OA
Causal Relationship Between Mood Swings and Non-Alcoholic Fatty Liver Disease Based on A Two-Sample Mendelian Randomization Study
目的:本研究基于孟德尔随机化(MR)研究设计,旨在揭示情绪波动与非酒精性脂肪性肝病(NAFLD)之间潜在的双向因果效应.方法:从全基因组关联研究(GWAS)数据中筛选与暴露(情绪波动)显著相关的单核苷酸多态性(SNP)位点作为工具变量(Ⅳs),依据设定的参数阈值进行提取.本研究将逆方差加权法(IVW)设定为主要分析方法,用于计算暴露与结局间的因果效应值;同时采用加权中位数法、MR Egger 回归、加权模式法和简单模型法进行补充分析,全面检验结果的稳健性与可靠性,评估情绪波动相关表型与非酒精性脂肪性肝病发生的关联.同时,进行敏感性分析,采用基因多效性检验、异质性检验和"留一法"检验评估结果的稳健性.结果:共筛选出 56 个与情绪波动相关的 SNP.IVW 分析显示情绪波动与非酒精性脂肪性肝病之间存在因果关系[OR=1.952 8,95%CI(1.021 0,3.735 1),P=0.043 1];Cochran's Q 检验结果(Q=48.790 1,df=55,P=0.709 4),表明不同SNP间无异质性;MR-PRESSO 分析未检出离群值(P=0.924 1);"留一法"分析结果表明单个SNP效应对因果关系无影响.结论:遗传预测的情绪波动与非酒精性脂肪性肝病发病风险存在正向因果关联,本研究为"情志伤肝"的中医理论提供了遗传学证据,但尚需更多研究进一步验证.
Objective:Based on Mendelian randomization(MR)study design,to reveal the potential bidirectional causal association between emotional fluctuation and non-alcoholic fatty liver disease(NAFLD)in this study.Method:Single nucleotide polymorphisms(SNPs)significantly associated with the exposure(mood swings)were selected from genome-wide association study(GWAS)data as instrumental variables(Ⅳs),extracted based on predefined threshold parameters.The inverse variance weighted(IVW)method was used as the primary analysis to estimate causal effects between exposure and outcome,with supplementary analyses including the weighted median method,MR Egger regression,weighted mode method,and simple model method to fully validate result robustness and reliability,and to evaluate the association between emotional fluctuation-related phenotypes and NAFLD onset.Sensitivity analysis was conducted,including pleiotropy tests,heterogeneity tests and leave-one-out analysis to assess the robustness of the results.Result:A total of 56 SNPs associated with mood swings were identified.IVW analysis revealed a causal relationship between mood swings and non-alcoholic fatty liver disease[OR=1.9528,95%CI(1.021 0,3.735 1),P=0.043 1].Cochran's Q test results(Q=48.790 1,df=55,P=0.709 4)indicated no heterogeneity among the SNPs.MR-PRESSO analysis detected no outliers(P=0.924 1).Leave-one-out analysis demonstrated that no single SNP significantly influenced the causal relationship.Conclusion:Genetically predicted mood swings show a positive causal association with the risk of non-alcoholic fatty liver disease(NAFLD).This study provides genetic evidence supporting the traditional Chinese medicine theory that"emotional distress impairs the liver",but further research is needed for validation.
