目的 通过系统评价现有文献,分析A型肉毒毒素(BTA)注射对唇裂修复术(CLR)术后瘢痕的改善效果.方法 遵循系统评价与Meta分析优先报告条目(PRISMA)指南,在PubMed、Embase、Scopus、Web of Science及Cochrane图书馆进行系统文献检索.检索时限截至2025年12月.采用主题词与自由词结合的策略,以"A型肉毒毒素""唇裂""瘢痕"等为关键词构建检索式.由两名研究者独立进行文献筛选、数据提取与偏倚风险评估,分歧由两位研究者协商解决,必要时征询资深研究者意见.主要结局指标为视觉模拟评分法(VAS)、温哥华瘢痕量表(VSS)评分、瘢痕宽度、BTA或CLR相关并发症.结果 共5项研究(216例患者)符合纳入标准.4项研究针对婴儿期初次CLR,1项研究纳入需行CLR的年长患者.所有患者均于口轮匝肌注射BTA(剂量范围:1~2 U/kg),其中1项研究额外于口周肌群注射.4项评估瘢痕宽度的研究均设0.9%氯化钠注射液对照组,结果显示BTA组瘢痕宽度显著减小.5项研究中分别有3项和2项报道BTA注射可改善VAS、VSS评分;均未见BTA或CLR相关并发症报告.结论 现有研究支持BTA注射可改善CLR术后瘢痕,且并发症风险较低,未来需扩大样本量进一步验证.

Objective To systematically review existing literature and analyze the effect of botulinum toxin type A(BTA)injection on scar improvement after cleft lip repair(CLR).Methods Following the Preferred Reporting Items for Systematic Review and Meta-Analyses(PRISMA)guidelines,a systematic literature search was conducted in PubMed,Embase,Scopus,Web of Science,and the Cochrane Library.The search period was up to December 2025.A combination of MeSH terms and free-text words was used,with"botulinum toxin""cleft lip"and"scar"as keywords to construct the search strategy.Two investigators independently performed literature screening,data extraction,and risk of bias assessment;disagreements were resolved through discussion or arbitration by a third party.The primary outcome measures included Visual Analogue Scale(VAS)score,Vancouver Scar Scale(VSS)score,scar width,and complications related to BTA or CLR.Results A total of 5 studies(216 patients)met the inclusion criteria.Four studies focused on primary CLR during infancy,and one study included older patients requiring CLR.All patients received BTA injection into the orbicularis oris muscle(dose range:1-2 U/kg),with one study additionally injecting into the perioral muscles.All four studies that assessed scar width included a normal saline(0.9%sodium chloride injection)control group,and showed significantly reduced scar width in the BTA group.Among the five studies,three and two reported that BTA injection improved VAS and VSS scores,respectively.No BTA-or CLR-related complications were reported.Conclusion Current evidence supports that BTA injection can improve scar after CLR with a low risk of complications;future studies with larger sample sizes are warranted for further validation.
