目的 观察经颅强交流电刺激(Hi-tACS)治疗青少年双相抑郁(BDD)的临床疗效.方法 前瞻性选取2024年6月至2025年8月在贵州省第二人民医院住院治疗的41例青少年BDD患者为研究对象,均为女性;按照随机数字表法分为观察组20例和对照组21例,两组患者均接受富马酸喹硫平片口服治疗,观察组在此基础上接受Hi-tACS治疗(77.5 Hz、15mA),而对照组仅接受伪刺激治疗,均连续治疗20 d.分别在治疗前、治疗结束时和治疗结束后第4周评估和比较两组患者17项汉密尔顿抑郁量表(HAMD-17)、社会功能缺陷筛选量表(SDSS)和杨氏躁狂评定量表(YMRS)评分,记录治疗期间不良反应发生情况.结果 两组患者治疗前后各个时点HAMD-17、SDSS、YMRS评分比较,差异均有统计学意义(均P<0.05).治疗前两组患者HAMD-17评分比较差异无统计学意义(P>0.05),治疗结束时、治疗结束后第4周观察组HAMD-17评分均低于对照组(均P<0.01);治疗前及治疗结束时两组患者SDSS评分比较差异均无统计学意义(均P>0.05),治疗结束后第4周观察组SDSS评分低于对照组(P<0.05);治疗前、治疗结束时、治疗结束后第4周两组患者YMRS评分比较差异均无统计学意义(均P>0.05).治疗期间未观察到任何轻躁狂/躁狂转换事件,仅观察组有1例患者出现轻微的皮肤刺痛,在短暂休息后自行缓解,未影响后续治疗.结论 15 mA、77.5 Hz的Hi-tACS能明显改善青少年BDD患者的抑郁症状,并促进其社会功能恢复.

Objective To investigate the clinical efficacy of high-intensity transcranial alternating current stimulation(Hi-tACS)in adolescents with bipolar depressive disorder(BDD).Methods A total of 41 adolescent female patients with BDD hospitalized in the Second People's Hospital of Guizhou Province from June 2024 to August 2025 were prospectively selected as study subjects.They were randomly divided into a control group(n=21)and an observation group(n=20)using a random number table.Both groups received oral quetiapine fumarate tablets,while the observation group additionally underwent Hi-tACS treatment(77.5 Hz,15 mA)and the control group received only sham treatment.All patients received continuous treatment for 20 days.The 17-item Hamilton Depression Rating Scale(HAMD-17),Social Disability Screening Schedule(SDSS),and Young Mania Rating Scale(YMRS)scores were assessed and compared between the two groups of patients before treatment,at the end of treatment,and at the 4th week after treatment completion.Adverse reactions during the treatment period were recorded.Results Comparison of HAMD-17,SDSS,and YMRS scores at all time points before and after treatment in the two groups showed statistically significant differences(all P<0.05).There was no statistically significant difference in HAMD-17 score between the two groups before treatment(P>0.05).At the end of treatment and at the 4th week after treatment,the HAMD-17 scores of the observation group were lower than those of the control group(both P<0.01).There were no statistically significant differences in SDSS scores between the two groups before and at the end of treatment(both P>0.05),but the observation group had lower SDSS score than the control group at the 4th week after treatment(P<0.05).No statistically significant differences in YMRS scores were found between the two groups before treatment,at the end of treatment,or at the 4th week after treatment(all P>0.05).No hypomanic/manic conversion events were observed during the treatment period.Only one patient in the observation group reported mild skin stinging,which resolved spontaneously after a brief rest without affecting subsequent treatment.Conclusion Hi-tACS at 15 mA and 77.5 Hz can significantly improve depressive symptoms in adolescent BDD patients and promote the recovery of their social function.
