光谱CT多参数预测结直肠癌神经侵犯的价值OA
Spectral CT multi-parameter quantitative prediction of perineural invasion in colorectal cancer
目的 探讨双层探测器光谱CT多参数预测结直肠癌神经侵犯(PNI)的临床价值.方法 回顾性收集2024年3月至2025年4月在浙江中医药大学附属第一医院(浙江省中医院)行二代双层探测器光谱CT腹部增强扫描检查且经病理检查证实的103例结直肠癌患者,根据是否发生PNI分为PNI(+)组(59例)和PNI(-)组(44例).收集所有患者动脉期、静脉期病灶单能级图像40、70、90 keV的CT值(CT40keV、CT70keV、CT90keV)、碘浓度(IC)、有效原子序数(Zeff)、电子云密度及同层面大动脉的CT值,并计算标准化碘浓度(NIC)和能谱曲线斜率(λHu),比较两组患者能谱参数.采用二元logistic回归分析结直肠癌PNI的独立预测因素并构建能谱多参数预测模型,绘制ROC曲线评估各项参数及联合模型对PNI的诊断效能.结果 PNI(+)组和PNI(-)组动脉期病灶IC、NIC、Zeff、λHu及CT40keV、CT70keV,静脉期病灶IC、NIC、λHu、CT40keV、CT90keV等参数比较,差异均有统计学意义(均P<0.05).二元logistic回归结果显示动脉期病灶λHu、静脉期病灶IC、静脉期病灶NIC为结直肠癌PNI的独立预测因素(均P<0.05).各因素单独预测结直肠癌PNI的AUC分别为0.731、0.614及0.550,这些参数构建的联合模型诊断结直肠癌PNI的灵敏度为0.804,特异度为0.795,AUC为0.845.结论 双层探测器光谱CT多参数可在术前预测结直肠癌PNI,为临床提供一种新的术前评估方式.
Objective To evaluate the clinical value of dual-layer detector spectral CT multi-parameters in predicting perineural invasion(PNI)in colorectal cancer(CRC).Methods A total of 103 patients with pathologically confirmed CRC who underwent abdominal contrast-enhanced scanning using dual-layer detector spectral CT at the First Affiliated Hospital of Zhejiang Chinese Medical University(Zhejiang Provincial Hospital of Traditional Chinese Medicine)from March 2024 to April 2025 were retrospectively enrolled.Based on PNI status,patients were divided into PNI-positive[PNI(+)](n=59)and PNI-negative[(PNI(-)](n=44)groups.CT values for monoenergetic images at 40,70,and 90 keV(CT40keV,CT70keV,CT90keV),iodine concentration(IC),effective atomic number(Zeff),electron density,and CT values of the large artery at the same level were measured in arterial and venous phases.Normalized IC(NIC)and spectral curve slope(λHu)were calculated.Spectral parameters were compared between the PNI(+)and PNI(-)groups.Binary logistic regression analysis was then used to identify independent predictors of PNI in CRC so as to construct a multi-parameter spectral prediction model.ROC curves were plotted to evaluate the diagnostic performance of individual parameters and the combined model for PNI.Results Statistically significant differences were observed between the PNI(+)and PNI(-)groups in arterial phase parameters including IC,NIC,Zeff,λHu,CT40keV,and CT70keV,as well as in venous phase parameters including IC,NIC,λHu,CT40keV,and CT90keV(all P<0.05).Binary logistic regression analysis showed that arterial phase λHu,venous phase IC,and venous phase NIC were independent predictors of PNI in CRC(all P<0.05).The AUC values for these individual parameters in predicting CRC PNI were 0.731,0.614,and 0.550,respectively.The combined model constructed using these parameters achieved a sensitivity of 0.804,specificity of 0.795,and an AUC of 0.845 for diagnosing PNI in CRC.Conclusion Dual-layer spectral CT multi-parameter analysis can predict PNI in CRC preoperatively,providing a novel approach for preoperative assessment.
