A Comparative Study of Arthroscopic Modified Mason-Allen Suture versus Suture Bridge Technique for Moderate Rotator Cuff Tears

目的 比较关节镜下改良Mason-Allen缝合与缝线桥技术治疗中型肩袖撕裂的临床疗效.方法 回顾性选取2022年1月至2024年5月北京市平谷区医院收治的110例中型肩袖撕裂患者的临床资料,根据肩袖固定方式的不同将患者分为缝线桥组(54例)和改良Mason-Allen组(56例).所有患者均行关节镜下肩袖修复术.通过比较两组术前及术后的疼痛视觉模拟评分、Constant-Murley评分评估患者的肩关节功能恢复情况,结合磁共振成像检查,比较两组患者肩袖总愈合率.结果 两组患者术后3个月疼痛视觉模拟评分均较术前显著降低(P<0.05),但两组间比较差异无统计学意义(P>0.05).两组患者术后12个月Constant-Murley量表中疼痛、日常生活活动、肩关节活动范围及肌力等4个维度评分及总分均显著高于术前(P<0.05),但两组间各维度评分及总分比较差异均无统计学意义(P>0.05).治疗后3个月,两组患者肩袖总愈合率比较,差异无统计学意义(缝线桥组92.59%vs 改良Mason-Allen组94.64%,P>0.05).结论 对于中型肩袖撕裂,关节镜下改良Mason-Allen缝合技术是一种安全、有效的治疗选择,且与缝线桥技术具有相似的临床疗效.

Objective To compare the clinical efficacy of arthroscopic modified Mason-Allen suture and suture bridge technique for the treatment of moderate rotator cuff tears.Methods A retrospective analysis was conducted on the clinical data of 110 patients with moderate rotator cuff tears admitted to Beijing Pinggu District Hospital from January 2022 to May 2024.According to the different rotator cuff fixation methods,they were divided into the suture bridge group(54 patients)and the modified Mason-Allen group(56 patients).All patients underwent arthroscopic rotator cuff repair.Shoulder function recovery was evaluated by comparing preoperative and postoperative visual analogue scale(VAS)scores and Constant-Murley scores between the two groups.The total rotator cuff healing rate was compared using magnetic resonance imaging.Results The VAS scores of both groups at 3 months postoperatively were significantly lower than those before surgery(P<0.05),but there was no statistically significant difference between the two groups(P>0.05).At 12 months postoperatively,the scores of the four dimensions(pain,activities of daily living,range of motion,and muscle strength)and the total score of the Constant-Murley scale in both groups were significantly higher than those before surgery(P<0.05),and there were no statistically significant differences between the two groups in any dimension or the total score(P>0.05).At 3 months after treatment,there was no statistically significant difference in the total rotator cuff healing rate between the two groups(suture bridge group:92.59%vs modified Mason-Allen group:94.64%,P>0.05).Conclusion For moderate rotator cuff tears,arthroscopic modified Mason-Allen suture is a safe and effective treatment option,with clinical efficacy similar to that of the suture bridge technique.
