iRootSP单尖充填法与两种热牙胶垂直加压法应用于窦道型慢性根尖周炎一次性根管治疗的临床疗效OA
Comparison of Clinical Efficacy of iRoot SP Single-Cone Obturation and Two Warm Gutta-Percha Vertical Compaction Techniques in Single-Visit Root Canal Treatment for Chronic Apical Periodontitis with Sinus Tract
目的 比较iRootSP单尖充填法、iRootSP糊剂热牙胶垂直加压与AH-Plus糊剂热牙胶垂直加压应用于一次性根管治疗的临床疗效.方法 回顾性选取2022年1月至2025年2月邯郸市眼科医院(邯郸市第三医院)收治的慢性根尖周炎患者155例,根据治疗方式的不同分为Ⅰ组(50例,采用iRootSP单尖充填法)、Ⅱ组(52例,采用iRootSP糊剂热牙胶垂直加压法)、Ⅲ组(53例,采用AH-Plus糊剂热牙胶垂直加压法).比较3组操作用时、填充效果、疼痛程度、窦道愈合情况、咀嚼功能、牙周指标、疗效及并发症发生情况.结果 Ⅰ组操作用时低于Ⅱ组、Ⅲ组(P<0.05);Ⅱ组操作用时低于Ⅲ组(P<0.05).3组视觉模拟评分法评分均随术后时间点延长降低(P<0.05);术后评估时间点与组别有交互作用(P<0.05);Ⅰ组术后3 d、术后1周视觉模拟评分法评分低于Ⅱ组、Ⅲ组(P<0.05),Ⅱ组术后3 d、术后1周视觉模拟评分法评分低于Ⅲ组(P<0.05).3组术后4周咀嚼效率高于术前(P<0.05);I组术后4周咀嚼效率[(76.03±8.12)%]高于Ⅱ组、Ⅲ组[(63.06±6.64)%、(60.45±6.23)%](P<0.05).3组术后4周牙龈、菌斑指数低于术前(P<0.05);Ⅰ组术后4周牙龈、菌斑指数低于Ⅱ组、Ⅲ组(P<0.05).结论 iRoot SP单尖充填法较iRootSP糊剂热牙胶垂直加压与AH-Plus糊剂热牙胶垂直加压在操作用时、减轻术后早期疼痛、促进咀嚼功能和牙周指标恢复方面表现出优势,在填充效果、窦道愈合情况、疗效和总体安全性上效果相当.
Objective To compare the clinical efficacy of iRoot SP single-cone obturation,iRoot SP paste with warm gutta-percha vertical compaction,and AH-Plus paste with warm gutta-percha vertical compaction in single-visit root canal treatment.Methods A retrospective study was conducted involving 155 patients with chronic apical periodontitis admitted to Handan Eye Hospital(Handan Third Hospital)from January 2022 to February 2025.Based on different treatment modalities,patients were divided into three groups:Group Ⅰ(50 cases,treated with iRoot SP single-cone obturation),Group Ⅱ(52 cases,treated with iRoot SP paste combined with warm gutta-percha vertical compaction),and GroupⅢ(53 cases,treated with AH-Plus paste combined with warm gutta-percha vertical compaction).The operation time,filling effect,pain level,sinus tract healing,masticatory function,periodontal indicators,clinical efficacy,and complications were compared among the three groups.Results The operation time in GroupⅠwas significantly shorter than that in GroupsⅡandⅢ(P<0.05);the operation time in GroupⅡwas significantly shorter than that in GroupⅢ(P<0.05).Visual analogue scale scores in all three groups decreased over time postoperatively(P<0.05).There was a significant interaction between postoperative evaluation time and group(P<0.05).At 3 days and 1 week postoperatively,VAS scores in GroupⅠwere significantly lower than those in GroupsⅡand Ⅲ(P<0.05),and scores in GroupⅡwere significantly lower than those in Group Ⅲ(P<0.05).Masticatory efficiency at 4 weeks postoperatively was significantly improved in all groups compared with preoperative levels(P<0.05);at 4 weeks,masticatory efficiency in Group Ⅰ[(76.03±8.12)%]was significantly higher than that in Groups Ⅱ and Ⅲ[(63.06±6.64)%and(60.45±6.23)%,respectively](P<0.05).Gingival index and plaque index at 4 weeks postoperatively were significantly lower than preoperative values in all groups(P<0.05);at 4 weeks,both indices in GroupⅠwere significantly lower than those in GroupsⅡandⅢ(P<0.05).Conclusion Compared with iRoot SP paste combined with warm gutta-percha vertical compaction and AH-Plus paste combined with warm gutta-percha vertical compaction,iRoot SP single-cone obturation demonstrated advantages in operation time,reduction of early postoperative pain,and promotion of masticatory function and periodontal recovery,while achieving comparable outcomes in filling effect,sinus tract healing,clinical efficacy,and overall safety.
