老年脆性骨折患者血钙与血红蛋白水平的相关性分析OA
Association between serum calcium and hemoglobin in the elderly patients with fragility fractures
目的 分析老年椎体与非椎体脆性骨折患者血钙与血红蛋白(hemoglobin,HGB)水平的相关性,并统计低钙血症与贫血的患病率.方法 纳入 2020 年 1 月至 2023 年 8 月于西安交通大学附属红会医院住院的脆性骨折患者.根据骨折部位分为椎体骨折组和非椎体骨折(其他部位骨折,包括髋部、肱骨近端、前臂)组.采用 Pearson 相关及偏相关分析评估血钙与 HGB 水平的相关性.采用卡方检验比较椎体与非椎体骨折患者低钙血症和贫血患病率的差异.结果 本研究共纳入 14 786 例椎体骨折患者和 16 017 例非椎体骨折患者.椎体和非椎体骨折患者的平均血钙水平分别为 2.28 mmol/L 和 2.21 mmol/L,差异有统计学意义(P<0.000 1).椎体骨折患者的血清 HGB 水平显著高于非椎体骨折患者(P<0.000 1).在校正了潜在混杂因素后,所有患者的血钙浓度与 HGB 水平呈显著正相关(r=0.425,P<0.001),进一步分层分析发现,在椎体骨折与非椎体骨折患者中两者也是呈显著正相关.再将骨折部位也纳入混杂因素后,相关性系数降低(r=0.390),但血钙仍与 HGB 水平显著相关(P<0.001).此外,椎体骨折患者低钙血症和贫血的患病率明显低于非椎体骨折患者(P<0.000 1).结论 在老年脆性骨折患者中,血钙水平与HGB 水平呈显著正相关,骨折部位是影响因素之一.与椎体骨折患者相比,非椎体骨折患者的低钙血症和贫血患病率更高,提示应常规监测非椎体骨折患者血钙和 HGB 水平,以防低钙血症和贫血引起的潜在风险.
Objective To investigate the correlation between serum levels of calcium and hemoglobin(HGB)in the elderly patients with vertebral and non-vertebral fragility fractures,and analyze the prevalence of hypocalcemia and anemia.Methods The patients with fragility fractures who were hospitalized in Honghui Hospital,Xi'an Jiaotong University from January 2020 to August 2023 were enrolled in the study.Participants were categorized into a vertebral fracture group or a non-vertebral fracture group(including hip,proximal humerus and forearm fractures).Pearson cor-relation and partial correlation analysis were performed to evaluate the association between serum calcium level and HGB level.The chi-square test was used to compare the prevalence of hypocalcemia and anemia in patients with vertebral and non-vertebral fractures.Results Totally 14 786 patients with vertebral fractures and 16 017 patients with non-vertebral fractures were enrolled in the study.The mean serum calcium level was significantly higher in the vertebral fracture group(2.28 mmol/L)than in the non-vertebral fracture group(2.21 mmol/L,P<0.000 1).Serum HGB levels were significantly higher in the vertebral fracture group than in the non-vertebral fracture group(P<0.001).After adjustment for potential confounders,a significant positive correlation was observed between serum calcium and HGB levels in the overall cohort(r=0.425,P<0.001).This positive correlation remained significant in both fracture groups upon strati-fied analysis.When fracture site was additionally included as a confounding factor,the correlation coefficient decreased(r=0.390)but remained statistically significant(P<0.001).Moreover,the prevalences of hypocalcemia and anemia were significantly lower in the vertebral fracture group than the non-vertebral fracture group(P<0.000 1).Conclusion Among the elderly patients with fragility fractures,serum calcium level is positively correlated with HGB level,and frac-ture site appears to be a contributing factor.Patients with non-vertebral fractures exhibit a higher prevalence of hypocal-cemia and anemia than those with vertebral fractures.These findings underscore the importance of routine monitoring of serum calcium and HGB levels in non-vertebral fracture patients to mitigate potential risks associated with hypocalcemia and anemia.
