目的:探讨依达拉奉右莰醇联合替罗非班对急性脑梗死(ACI)患者脑血流动力学、氧化应激及血管内皮功能的改善效果.方法:回顾性选取 2023年1月~2024年12 月某院收治的114例 ACI 患者作为研究对象,根据治疗方案分为对照组(替罗非班治疗)和研究组(依达拉奉右莰醇联合替罗非班治疗),每组57例.比较两组患者氧化应激指标、脑血流动力学指标、血管内皮功能指标、神经功能指标、日常生活能力指标、临床疗效和不良反应发生情况.结果:重复测量方差分析显示,两组患者丙二醛(MDA)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、外周阻力(PR)、平均血流速度(Vmean)、脑血管阻力(CVR)、血栓素A2(TXA2)、内皮素-1(ET-1)、血管生成素-1(ANG-1)、巴氏指数(BI)、美国国立卫生研究院卒中量表(NIHSS)的时点、组间、交互效应比较均具有统计学差异(P<0.05).治疗1周和 2 周后,两组患者 MDA、PR、CVR、TXA2、ET-1、ANG-1、NIHSS 评分均降低,且研究组低于对照组(P<0.05);SOD、GSH-Px、BI 评分均升高,且研究组高于对照组(P<0.05);Vmean 均加快且研究组快于对照组(P<0.05).研究组患者临床治疗总有效率高于对照组(P<0.05).两组患者不良反应总发生率比较无统计学差异(P>0.05).结论:依达拉奉右莰醇联合替罗非班通过精准调控 ACI 核心病理环节,在改善脑血流动力学、减轻氧化应激及保护血管内皮功能方面疗效显著,且未增加不良反应发生风险.

Objective:To investigate the improving effects of edaravone dexborneol combined with tirofiban on cerebral hemodynamics,oxidative stress and vascular endothelial function in patients with acute cerebral infarction(ACI).Methods:A total of 114 ACI patients admitted to our hospital from January 2023 to December 2024 were retrospectively enrolled and divided into control group(treated with tirofiban)and study group(treated with edaravone dexborneol combined with tirofiban)according to treatment regimens,with 57 cases in each group.Oxidative stress indicators,cerebral hemodynamics indicators,vascular endothelial function indicators,neurological function indicators,daily living ability indicators,clinical efficacy and adverse reactions were compared between the two groups.Results:Repeated measurement analysis showed that there were statistically significant differences in time-point,inter-group and interaction effects of malondialdehyde(MDA),superoxide dismutase(SOD),glutathione peroxidase(GSH-Px),peripheral resistance(PR),mean blood flow velocity(Vmean),cerebrovascular resistance(CVR),thromboxane A2(TXA2),endothelin-1(ET-1),angiopoietin-1(ANG-1),Barthel index(BI)and National Institutes of Health Stroke Scale(NIHSS)score between the two groups(P<0.05).After 1 and 2 weeks of treatment,the levels of MDA,PR,CVR,TXA2,ET-1,ANG-1,and NIHSS scores decreased in both groups,and the study group showed lower values than the control group(P<0.05),and those in the study group were lower than those in the control group(P<0.05);SOD,GSH-Px,and BI score were increased,and those in the study group were higher than those in the control group(P<0.05);Vmean was increased,and that in the study group was faster than the control group(P<0.05).The total clinical effective rate of the study group was higher than that of the control group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Edaravone dexborneol combined with tirofiban can significantly improve cerebral hemodynamics,reduce oxidative stress and protect vascular endothelial function in patients with ACI,without increasing adverse reactions.
