银杏内酯联合尼莫地平治疗对高血压脑出血术后恢复的影响OA
Effect of Ginkgolide Combined with Nimodipine Treatment on the Postoperative Recovery of Patients with Hypertensive Cerebral Hemorrhage
目的:探讨银杏内酯联合尼莫地平对高血压脑出血(HICH)术后恢复的影响.方法:本研究采用回顾性研究,选取 2016 年1月~2018年12 月某院收治的 280 例 HICH 术后患者作为研究对象,按照治疗方法不同分为对照组(尼莫地平治疗)和观察组(银杏内酯联合尼莫地平治疗),每组140 例.比较两组患者神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]、血脑屏障功能(BBB)及血清生化因子[神经元特异性烯醇化酶(NSE)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)]水平及患者预后[格拉斯哥结局量表(GOS)评分],并统计不良反应发生情况.结果:两组患者 NIHSS 评分在时间因素、组别因素及时间与组别交互作用比较具有统计学差异(P<0.05).治疗3个月后,两组患者 NIHSS 评分、BBB 指数、NSE、TNF-α和 hs-CRP 水平均降低,且观察组低于对照组(P<0.05).观察组患者临床优良率(65.00%)高于对照组(50.71%,P<0.05).两组患者不良反应总发生率比较无统计学差异(P>0.05).结论:银杏内酯联合尼莫地平用于 HICH 术后患者可改善其神经功能、促进血脑屏障功能恢复,并有效降低血清 NSE 及炎症因子水平,提高远期预后优良率,且未增加不良反应风险,具有良好的安全性.
Objective:To explore the impact on the postoperative recovery of ginkgolides and nimodipine on patients with hypertensive intracerebral hemorrhage(HICH).Methods:A retrospective study was conducted in this study,and 280 patients after HICH in a hospital from January 2016 to December 2018 were selected as the research object.According to different treatment methods,they were divided into control group(nimodipine treatment)and observation group(ginkgolide combined with nimodipine treatment),with 140 patients in each group.The neurological function[scores of National Institutes of Health Stroke Scale(NIHSS)],blood brain barrier(BBB)and Serum biochemical factors[serum neuron specific enolase(NSE)were compared between the two groups.NSE),tumor necrosis factor-α(TNF-α),high sensitivity C reactive protein(hs-CRP)]and patient prognosis[Glasgow Outcome Scale(GOS)scores],and the incidence of adverse reactions was counted.Results:The NIHSS scores of the two groups were statistically different in time factor,group factor and interaction between time and group(P<0.05).After 3 months of treatment,the NIHSS score,BBB index,NSE,TNF-α and hs-CRP levels in the two groups decreased,and the observation group was lower than the control group(P<0.05).The excellent and good rate of patients in the observation group(65.00%)was higher than that in the control group(50.71%,P<0.05).There was no statistical difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Ginkgolide combined with nimodipine can improve the neurological function,promote the recovery of blood-brain barrier function,effectively reduce the levels of serum NSE and inflammatory factors,and improve the long-term excellent and good prognosis,without increasing the risk of adverse reactions,with good safety.
