基于"瘀毒损神、枢机不利"的形神共病观论治卒中后抑郁OA

Post-stroke depression(PSD)is a typical manifestation of form-spirit co-morbidity after stroke.Its high incidence and severe disability significantly hinder patient recovery and reduce quality of life.Based on the traditional Chinese medicine(TCM)theory of"form-spirit co-morbidity",this paper proposes the core pathogenesis hypothesis of"stasis-toxin damaging the spirit and impairment of pivotal function"in PSD:endogenous stasis-toxin serves as the pathological basis of form-spirit co-morbidity,impairment of pivotal function is the key link in the disharmony between form and spirit,and damage to the spirit-orifice constitutes the core mechanism of depressive manifestations.These three factors interact to form a vicious cycle,driving disease progression.Based on this pathogenesis,a TCM therapeutic system of"detoxifying and resolving stasis,regulating and smoothing pivotal function,and calming the spirit and stabilizing the mind"is established.This not only provides scientific support for TCM diagnosis and treatment of PSD but also offers new theoretical perspectives and practical approaches for its clinical management.
