目的:探讨老年终末期肾病(ESRD)患者脑影像学皮质结构改变与抑郁、消极应对方式之间的关系.方法:收集老年ESRD患者35例作为ESRD组,招募41例老年健康志愿者作为对照组.2组均行全脑3D-T1WI扫描并填写简易智力状态检查量表(MMSE)、9条目患者健康问卷(PHQ-9)和简易应对方式量表.应用FreeSurfer软件基于表面的形态学测量方法比较2组脑皮质厚度;以差异脑区皮质厚度为中介变量,应对方式评分为调节变量,在R语言平台bruceR包内进行有调节的中介分析.结果:与对照组相比,ESRD组的左侧楔前叶、双侧岛叶和双侧颞上回皮质厚度显著减少(均P<0.05);ESRD组整体皮质厚度,以及左侧楔前叶、左侧岛叶、左侧颞上回和右侧岛叶皮质厚度均与PHQ-9评分呈负相关(r=-0.435,PFDR<0.001;r=-0.503,PFDR<0.001;r=-0.423,PFDR<0.001;r=-0.391,PFDR<0.001;r=-0.240,PFDR=0.048).中介分析表明,左侧颞上回皮质厚度完全中介老年ESRD与抑郁的关系(β=-1.30,P>0.05),中高水平的消极应对方式在这一关系中起调节作用.结论:老年ESRD患者多个脑区皮质结构改变与抑郁相关,消极应对方式在左颞上回结构异常介导的抑郁间起调节作用,该结果可为老年ESRD患者的治疗干预提供潜在的神经心理标记.

Objective:To explore the relationship between cortical structural changes in brain imaging and depression and negative coping styles in elderly patients with end-stage renal disease(ESRD).Methods:A total of 35 elderly ESRD patients were included in the ESRD group,whereas another 41 older healthy volunteers were included in the control group.All participants underwent 3D-T1WI scans and were evaluated to complete MMSE,PHQ-9 and simple coping styles questionnaire.The analysis compared cortical thickness differences between the two groups using surface-based morphometry measurements with FreeSurfer software.Using the cortical thickness in the different brain regions as the mediating variable and the coping style score as the moderating variable,a moderated mediation analysis was conducted within the bruceR package of the R language platform.Results:Compared with the control group,the cortical thickness in the left precuneus,bilateral insula and bilateral superior temporal gyri decreased in the ESRD group(all P<0.05).The overall cortical thickness and that in the left precuneus,left insula,left superior temporal gyrus and right insula were negatively correlated with PHQ-9 score(r=-0.435,PFDR<0.001;r=-0.503,PFDR<0.001;r=-0.423,PFDR<0.001;r=-0.391,PFDR<0.001;r=-0.240,PFDR=0.048).Mediation analysis showed that the cortical thickness in the left superior temporal gyrus completely mediated between ESRD and depression(β=-1.30,P>0.05),and an intermediate-high level of negative coping style played a moderating role in this relationship.Conclusions:Cortical structural changes in multiple brain regions of elderly ESRD patients are associated with depression.Negative coping style plays a regulatory role in depression mediated by structural abnormalities in the left superior temporal gyrus.This result may provide potential neuro-psychological markers for the treatment intervention of elderly ESRD patients.
