维持性血液透析伴认知功能障碍患者脑功能连接密度改变与全局神经血管解耦的分析OA
Changes in functional connectivity density and global neurovascular coupling in maintenance hemodialysis patients with cognitive dysfunction
目的:探讨维持性血液透析(MHD)伴认知功能障碍患者的脑功能连接密度(FCD)和全局神经血管耦合(NVC)变化,以及临床与病理生理机制.方法:29例MHD伴认知功能障碍患者(MHD组)和29例健康志愿者(对照组)行静息态fMRI扫描,计算全局功能连接密度(gFCD)、长程功能连接密度(lFCD)和短程功能连接密度(sFCD),并通过脑血流量(CBF)与FCD的相关性评估全局NVC.结果:与对照组相比,MHD组左侧海马、海马旁回、梭状回及苍白球的gFCD增高,右侧海马、海马旁回和丘脑的lFCD增高,左侧海马、海马旁回、梭状回、苍白球及颞下回的sFCD增高,而左侧顶下缘角回、缘上回和中央后回的sFCD减低.与对照组相比,MHD组CBF-sFCD全局耦合系数显著降低(P<0.05).MHD组右侧丘脑lFCD与血红蛋白水平呈负相关,右侧海马lFCD与语言功能评分呈负相关;左侧缘上回sFCD与血红蛋白呈正相关;左侧顶下缘角回sFCD与视觉空间功能评分呈负相关;CBF-gFCD和CBF-sFCD全局耦合系数与语言功能评分呈正相关(均P<0.05).结论:MHD伴认知功能障碍患者存在广泛的脑功能连接异常,全局NVC的解耦可能在认知功能障碍中起重要作用.
Objective:To investigate the changes in functional connectivity density(FCD)and global neurovascular coupling(NVC)in maintenance hemodialysis(MHD)patients with cognitive dysfunction,and to explore their clinical and pathophysiological mechanisms.Methods:Resting-state fMRI was performed in 29 MHD patients with cognitive dysfunction(MHD group)and 29 healthy controls(contorol group).Global FCD(gFCD),long-range FCD(lFCD),and short-range FCD(sFCD)were calculated.Global NVC was assessed by the correlation between cerebral blood flow(CBF)and FCD.Results:Compared with the control group,the MHD group showed the increased gFCD in the left hippocampus,parahippocampal gyrus,fusiform gyrus and globus pallidus,the increased lFCD in the right hippocampus,parahippocampal gyrus and thalamus,the increased sFCD in the left hippocampus,parahippocampal gyrus,fusiform gyrus,globus pallidus and inferior temporal gyrus,and the decreased sFCD in the left angular gyrus,supramarginal gyrus and postcentral gyrus.Compared with the control group,the global coupling coefficient of CBF-sFCD in the MHD group significantly decreased(P<0.05).In the MHD group,the lFCD in the right thalamus was negatively correlated with hemoglobin,and the lFCD in the right hippocampus was negatively correlated with the language function score,the sFCD in the left supramarginal gyrus was positively correlated with hemoglobin,and the sFCD in the left angular gyrus was negatively correlated with visuospatial function score,and the global coupling coefficients of CBF-gFCD and CBF-sFCD were positively correlated with language function score(both P<0.05).Conclusions:MHD patients with cognitive dysfunction exhibit widespread abnormalities in functional connectivity,and the decoupling of global NVC may play a critical role in cognitive dysfunction.
