Analysis of spontaneous brain activity changes in patients with spinal cord injury using resting-state fMRI

目的:应用静息态fMRI探讨脊髓损伤患者自发性脑活动的变化情况.方法:前瞻性收集43例脊髓损伤患者(脊髓损伤组),招募年龄、性别及受教育程度相匹配的健康志愿者43例作为对照组.2组均行静息态fMRI扫描,行局部一致性(ReHo)、分数低频振幅(fALFF)分析,采用两样本t检验比较组间差异.结果:与对照组相比,脊髓损伤组呈现脑功能重组:右侧额中回、Brodmann 9区的ReHo值显著升高,双侧眶部额内侧回、Brodmann 10区、左侧眶部额下回、左侧岛叶的ReHo值显著降低;右侧顶上回、右侧中央后回fALFF值显著升高,右侧楔前叶、左侧后扣带回fALFF值显著降低.结论:脊髓损伤患者感觉运动网络与认知控制相关脑区自发性神经元活动的改变,为理解脊髓损伤后的神经可塑性提供了重要的影像学依据.

Objective:To investigate the spontaneous brain activity changes in patients with spinal cord injury by resting-state fMRI.Methods:43 patients with spinal cord injury were prospectively enrolled as the injury group,and 43 healthy volunteers matched in age,sex and education level were recruited as the control group.All subjects underwent resting-state fMRI,and regional homogeneity(ReHo)and fractional amplitude of low-frequency fluctuation(fALFF)analyses were performed,respectively.Two-sample t-test was used to compare the differences between groups.Results:Compared with the control group,the injury group showed brain functional reorganization.The ReHo values in the right middle frontal gyrus and Brodmann 9 area increased significantly,while the ReHo values in the bilateral orbital medial frontal gyri,Brodmann area 10,the left orbital inferior frontal gyrus and left insula decreased significantly.The fALFF values of the right superior parietal gyrus and the right postcentral gyrus increased significantly,while the fALFF values of the right precuneus and left posterior cingulate gyrus significantly decreased.Conclusion:The changes in spontaneous neuronal activity in the brain regions related to sensory-motor networks and cognitive control in patients with spinal cord injury provide important imaging evidence for understanding the neural plasticity after spinal cord injury.
