Application value of transcutaneous ultrasound combined with fistula contrast-enhanced ultrasound in preoperative evaluation of complex anal fistula

目的:探讨经体表超声联合瘘管超声造影在复杂性肛瘘术前评估中的应用价值.方法:采用前瞻性对照研究设计,选取复杂性肛瘘患者80例,术前均行超声检查,其中40例行常规经体表超声联合腔内超声检查(常规超声组),40例行经体表超声联合瘘管超声造影检查(超声造影组).记录2组主瘘管、支瘘管、脓腔、内口位置和数目及Parks分型,以手术结果作为参照,分析2种超声检查方法的诊断结果.结果:2组在主瘘管、内口、脓腔的诊断符合率上差异均无统计学意义(均P>0.05).超声造影组与常规超声组支瘘管的诊断符合率分别为93.5%(86/92)、83.5%(76/91),差异有统计学意义(P<0.05).超声造影组与常规超声组复杂性肛瘘Parks分型的诊断符合率分别为87.5%(35/40)和92.5%(37/40),差异无统计学意义(P>0.05).结论:经体表超声联合瘘管超声造影可清晰显示复杂性肛瘘的内口、主瘘管、支瘘管及脓腔结构,并精准分型,对支瘘管的诊断准确性较常规超声高,尤其适用于无法行腔内检查的复杂性肛瘘,为临床治疗提供更多依据.

Objective:To explore the application value of transcutaneous ultrasound combined with fistula contrast-enhanced ultrasound(CEUS)in the preoperative evaluation of complex anal fistulas.Methods:A prospective controlled study design was employed.A total of 80 patients with complex anal fistulas confirmed by postoperative pathology were selected.Preoperative ultrasound examinations were performed.Among them,40 cases underwent conventional transcutaneous ultrasound combined with endocavitary ultrasound examination(conventional ultrasound group),and 40 cases underwent transcutaneous ultrasound combined with fistula CEUS(CEUS group).The main fistula,branch fistula,location and number of internal openings,abscess cavity,and Parks classification were recorded.The surgical results were used as the reference standard to analyze the diagnosis results of the two ultrasound methods.Results:There were no statistically significant differences between the two groups in the diagnostic accordance rate of internal openings,main fistulas,or abscess cavities(all P>0.05).The diagnostic accordance rate for branch fistulas in the CEUS group and the conventional ultrasound group were 93.5%(86/92)and 83.5%(76/91),respectively,with a statistically significant difference(P<0.05),while the diagnostic accordance rate for Parks classification of complex anal fistulas were 87.5%(35/40)and 92.5%(37/40),respectively,with no statistically significant difference(P>0.05).Conclusions:Transcutaneous ultrasound combined with fistula CEUS can clearly display the internal opening,main fistula,branch fistula,and abscess cavity structures of complex anal fistulas and achieve precise classification.It shows high diagnostic accuracy for branch fistulas compared with conventional ultrasound.For complex anal fistulas that cannot undergo endocavitary examination,transcutaneous ultrasound combined with fistula CEUS can be performed to provide more evidence for clinical treatment.
