目的 探究真菌性鼻窦炎(fungal rhino-sinusitis,FRS)患儿血清 C 型凝集素-1(Dectin-1)、β-防御素-3(β-defensin-3,HBD-3)、胸腺基质淋巴细胞生成素(thymic stromal lymphopoietin,TSLP)水平及其诊断价值.方法 选取 68 例 2022 年 5 月—2024 年 3 月在邯郸市第一医院就诊的 FRS 患儿作为病例组,同期选取 68 例进行治疗的非 FRS 患儿作为对照组.另选取在该院就诊的 2024 年 5 月—2025 年 5 月 45 例 FRS 患儿(FRS 组)及进行治疗的 45 例非 FRS 患儿(非 FRS 组)对诊断模型进行外部验证.采用 ELISA 法检测血清 Dectin-1、HBD-3、TSLP 表达水平;多因素logistic回归分析影响FRS发生的相关因素;ROC 曲线分析血清Dectin-1、HBD-3、TSLP 水平对FRS的诊断价值.结果 病例组 C 反应蛋白、白细胞计数、中性粒细胞百分比、血清 Dectin-1、TSLP 水平均显著高于对照组,血清 HBD-3水平显著低于对照组(P<0.05).血清 Dectin-1、HBD-3、TSLP、C 反应蛋白、白细胞计数、中性粒细胞百分比是影响FRS 发生的相关因素(P<0.05);血清 Dectin-1、HBD-3、TSLP 联合诊断 FRS 的曲线下面积(AUC)为 0.949,优于各自单一诊断(Z三者联合-Dectin-1=3.853、Z三者联合-HBD-3=3.116,Z三者联合-TSLP=3.512,P<0.001、P=0.002、P<0.001),联合诊断的敏感度和特异度分别为 88.24%、89.71%.三者联合诊断模型在外部验证中的 AUC 为 0.945,敏感度为 91.11%,特异度为 84.44%.结论 血清 Dectin-1、TSLP 水平在FRS 患儿中明显升高,而血清HBD-3 水平明显降低,三者联合对FRS的发生有更高的诊断价值.

Objective To investigate the levels and diagnostic value of serum Dectin-1,β-defensin-3(HBD-3),thymic stromal lymphopoietin(TSLP)in children with fungal rhino-sinusitis(FRS).Methods From May 2022 to March 2024,68 FRS children from the Handan First Hospital were selected as the case group,and another 68 non FRS children were selected as the control group.Another 45 children with FRS(FRS group)who were treated in the hospital from May 2024 to May 2025 and 45 children without FRS(non-FRS group)who received treatment were selected for external validation of the diagnostic model.ELISA method was applied to detect the expression levels of serum Dectin-1,HBD-3,and TSLP.Multivariate logistic regression was applied to analyze the relevant factors affecting the occurrence of FRS.ROC curve was applied to analyze the diagnostic value of serum Dectin-1,HBD-3,and TSLP levels for FRS.Results The C-reactive protein white blood cell count,neutrophil percentage,serum Dectin-1,TSLP levels in the case group were obviously higher than those in the control group,while serum HBD-3 level was obviously lower than that in the control group(P<0.05).Serum Dectin-1,HBD-3,TSLP,C-reactive protein,white blood cell count,and neutrophil percentage were related factors affecting the occurrence of FRS(P<0.05);the area under the curve(AUC)of the combination of serum Dectin-1,HBD-3,and TSLP in the diagnosis of FRS was 0.949,which was better than their respective single diagnoses(Z triple combination-Dectin-1=3.853,Z triple combination-HBD-3=3.116,Z triple combination-TSLP=3.512,P<0.001,P=0.002,P<0.001),the sensitivity and specificity of the combined diagnosis were 88.24%and 89.71%,respectively.The AUC of the combined diagnostic model of the three in external validation was 0.945,with a sensitivity of 91.11%and a specificity of 84.44%.Conclusion Serum Dectin-1 and TSLP levels are obviously elevated in children with FRS,while serum HBD-3 level is obviously decreased.The combination of the three has higher diagnostic value for the occurrence of FRS.
