11例西弗射盾子囊霉致耳道真菌感染的鉴定与药敏分析OA
Identification and antifungal susceptibility analysis of 11 cases of otomycosis caused by Stephanoascus ciferrii
目的 分析外耳道炎患者中分离的西弗射盾子囊霉的鉴定及耐药特征,为少见真菌感染的诊疗提供科学依据.方法 2023 年 9 月 1 日—2025 年 9 月 30 日四川泰康医院送检 280 份耳道分泌物样本,对 11 例外耳道炎患者耳分泌物中分离出的西弗射盾子囊霉进行培养、质谱鉴定及 ITS 测序鉴定,并结合药敏结果进行分析.结果 所有菌株可在血琼脂平板、沙氏培养基上生长,念珠菌显色平板菌落呈蓝色、质地坚硬;显微镜下可见卵圆形孢子沿菌丝排列.质谱鉴定与ITS测序结果一致(99%相似度).药敏结果显示西弗射盾子囊霉对氟康唑的MIC值较高(>32 μg/mL),对两性霉素 B 表现出较低的 MIC 值.结论 西弗射盾子囊霉为罕见条件致病真菌,可致耳道感染.联合传统培养与分子鉴定可提高诊断准确率,为临床合理用药提供参考.
Objective To analyze the antifungal susceptibility and resistance characteristics of Stephanoascus ciferrii(S.ciferrii)isolated from patients with otitis externa,and to offer evidence for the diagnosis and treatment of infections caused by rare fungi.Methods Among 280 specimens submitted to Sichuan Taikang Hospital from September 1,2023 to September 30,2025,eleven S.ciferrii strains were isolated from ear discharge samples of patients with otitis externa.The isolates were cultured and identified by MALDI-TOF MS and ITS sequencing,and antifungal susceptibility testing(AFST)was performed.Results All isolates could grow on blood agar and Sabouraud dextrose agar.On Candida CHROMagar,colonies appeared blue and firm.Microscopy revealed oval conidia arranged along the hyphae.MALDI-TOF MS identification was consistent with ITS sequencing(99%sequence identity).The isolates had high MICs to fluconazole(>32 µg/mL),whereas and relatively lower MICs to amphotericin B.Conclusion S.ciferrii is a rare opportunistic pathogen that can cause ear canal infections.Combining conventional culture with molecular identification improves diagnostic accuracy and may assist in guiding rational antifungal therapy.
