目的 推动医院抗菌药物使用强度达标.方法 选取某三级甲等综合医院 28 个临床科室 2022 年 1 月至 2024 年 6 月出院病历首页数据,计算各科室病例组合指数(CMI)和抗菌药物平均日剂量(DDDs).采用中位值法设定 DDDs 区间考核目标值,以 CMI 和DDDs 分别为横、纵坐标,以(1,34)为坐标交点构建波士顿矩阵,并根据结果对各临床科室进行管理,并动态评估管理前(2022 年,采用单一目标值)和管理后(2023 年及 2024 年上半年,采用区间考核目标值)对各临床科室 DDDs 实际管控成效.结果 与 2022 年比较,2023 年和 2024 年上半年全院 DDDs 分别下降 3.75、4.13,CMI 分别提升 0.01、0.04.全院 DDDs 与 CMI 均逐步接近目标值.波士顿矩阵分析结果表明,与管理前比较,管理后Ⅰ象限(放任区)临床科室数量保持不变(仍为 7 个),Ⅱ象限(严管区)由 7 个减至 4 个,Ⅲ象限(鼓励区)由 10 个增至 11 个,Ⅳ象限(表扬区)由 4 个增至 6 个.结论 建立的波士顿矩阵可用于抗菌药物 DDDs 动态评价,有助于指导抗菌药物使用强度的管理与优化,促进医院各临床科室合理使用抗菌药物.

Objective To promote the compliance of antimicrobial use density(AUD)in hospitals.Methods Data from the front pages of discharge cases of 28 clinical departments of a tertiary first-class general hospital from January 2022 to June 2024 were collected.The case mix index(CMI)and defined daily doses(DDDs)of antimicrobials were calculated for each department.The median method was used to establish the interval assessment target values for DDDs.A Boston matrix was constructed with CMI value as the horizontal axis and DDDs value as the vertical axis,using(1,34)as the coordinate intersection point,each clinical department was managed based on the results,and the actual control effectiveness of DDDs in each clinical department was dynamically evaluated before management implementation(2022,using a single target value)and after management implementation(2023 and the first half of 2024,using interval assessment target values).Results Compared with 2022,the overall hospital DDDs value decreased by 3.75 and 4.13 in 2023 and the first half of 2024,respectively,while the CMI value increased by 0.01 and 0.04,respectively.Both the overall hospital DDDs and CMI gradually approached the target values.Boston matrix analysis showed that,compared with before management implementation,the number of clinical departments in Quadrant I(laissez-faire zone)remained unchanged(still seven departments),the number in Quadrant Ⅱ(strict control zone)decreased from seven to four,the number in Quadrant Ⅲ(encouragement zone)increased from 10 to 11,and the number in Quadrant Ⅳ(commendation zone)increased from four to six after management implementation.Conclusion The established Boston matrix can be used for dynamic evaluation of interval assessment of antimicrobial DDDs,helping to guide the management and optimization of AUD and promoting rational drug use in clinical departments of the hospital.
