基于糖脂代谢紊乱与衰老双向调控的临床干预进展OA
Clinical intervention progress based on the bidirectional regulation of glycolipid metabolic disorders and aging
随着人口老龄化进程持续加速,衰老已成为目前重要的公共健康问题.深入解析衰老的核心调控机制、探索科学有效的干预策略已成为当下研究的热点.糖脂代谢紊乱并非衰老的单纯伴随现象,而是与衰老形成双向调控的恶性循环:衰老进程中代谢调控网络功能衰退,可诱发胰岛素抵抗、线粒体功能异常等,进而导致糖脂代谢稳态失衡;而糖脂代谢重塑又通过加剧氧化应激、激活慢性炎症、诱导细胞衰老等途径,加速机体组织器官功能衰退,推动心血管疾病、2型糖尿病、代谢功能障碍相关脂肪性肝病等衰老相关疾病的发生发展.本文基于二者双向调控的核心分子机制,系统梳理靶向糖脂代谢的相关临床干预研究进展,包括生活方式调整(饮食、运动、睡眠)、经典及新型药物治疗(二甲双胍、胰高血糖素样肽-1受体激动剂、钠-葡萄糖协同转运蛋白2抑制剂等)及新兴靶向疗法,以期为衰老相关疾病的精准防治提供循证依据与临床转化思路.
As the aging of the population continues to accelerate,aging has become an important public health issue nowadays.An in-depth analysis of the core regulatory mechanisms of aging and exploring scientifically effective interventional strategies have become the key focus in current research.Disorders of glucose and lipid metabolism are not only a core metabolic feature of the aging process,but also form a vicious cycle of bidirectional regulation with the aging process.Age-related dysfunction of the metabolic regulatory network can induce insulin resistance,mitochondrial dysfunction,and other abnormalities,ultimately disrupting the homeostasis of glucose and lipid metabolism.Conversely,metabolic remodeling further accelerates the decline of tissue and organ function by intensifying oxidative stress,activating chronic inflammation,and inducing cellular senescence.These processes,in turn,drive the onset and progression of age-related diseases including cardiovascular disease,type 2 diabetes,and metabolic dysfunction-associated fatty liver disease(MAFLD).Based on the bidirectional interaction between aging and metabolic dysregulation,this paper systematically reviews clinical intervention studies targeting glucose and lipid metabolism through key bidirectional molecular mechanisms.The review encompasses lifestyle modifications(diet,exercise,sleep),classical and novel pharmacotherapies(metformin,glucagon-like peptide-1 receptor agonists,sodium-glucose cotransporter 2 inhibitors,etc),and emerging targeted therapies.These insights aim to provide evidence-based guidance and clinical translation strategies for the precision prevention and treatment of age-related diseases.
