目的 评估超灵敏电化学发光尿液脂阿拉伯甘露聚糖(lipoarabinomannan,LAM)检测技术在 HIV阴性的疑似肺结核个体中的诊断准确性.方法 选取2025年1月至5月在首都医科大学附属北京胸科医院收治的 HIV阴性的疑似肺结核患者(≥18岁).所有参与者均提供痰液样本用于涂片镜检、Xpert MTB/RIF检测和分枝杆菌培养,同时提供尿液样本用于超灵敏尿液LAM 检测.通过肺结核的综合诊断评估其诊断性能.结果 本研究收集280例 HIV阴性疑似肺结核成人的尿液样本.其中,144例(51.4%)为病原学诊断阳性病例,64例(22.9%)为临床诊断病例,72例(25.7%)被判定为非结核患者.以综合诊断为诊断金标准,尿液检测的敏感度51.44%(95%CI为44.65%~58.23%),特异度98.61%(95%CI为95.91%~100.00%).尿液LAM 检测联合痰Xpert检测的敏感度[76.08%(95%CI为70.29%~81.86%)]高于单独尿液LAM 检测.结论 超灵敏尿液LAM 检测展现出良好的诊断准确性,有望为 HIV阴性肺结核患者提供快速的结核病诊断方案.

Objective To evaluate the diagnostic accuracy of an ultrasensitive electrochemiluminescence assay for detecting urinary lipoarabinomannan(LAM)in HIV-negative individuals with presumptive pulmonary tuberculosis.Methods HIV-negative patients(≥18 years)with suspected pulmonary tuberculosis admitted to Beijing Chest Hospital,Capital Medical University from January to May 2025.Sputum samples from all participants were subjected to smear microscopy,Xpert MTB/RIF assay,and mycobacterial culture.Concurrent urine samples were tested using the ultrasensitive LAM assay.Diagnostic performance was assessed against a composite reference standard for pulmonary tuberculosis diagnosis.Results Urine samples were collected from 280 HIV-negative adults with suspected pulmonary tuberculosis.Among them,144(51.4%)were microbiologically confirmed,64(22.9%)were clinically diagnosed,and 72(25.7%)were classified as non tuberculosis patients.Taking comprehensive diagnosis as the gold standard,the urinary LAM assay demonstrated a sensitivity of 51.44%(95%CI:44.65%-58.23%)and a specificity of 98.61%(95%CI:95.91%-100.00%).The sensitivity of combined urine LAM and sputum Xpert testing[76.08%(95%CI:70.29%-81.86%)]was higher than that of urine LAM testing alone.Conclusions The ultrasensitive urine LAM assay shows promising diagnostic accuracy and could offer a rapid tuberculosis diagnostic solution for HIV-negative patients with pulmonary tuberculosis.
