The tumor immune microenvironment of endometrial cancer based on molecular subtypes
子宫内膜癌位列女性三大妇科恶性肿瘤之列,主要侵袭绝经期后女性群体,在全球范围内发病趋势持续攀升.早期确诊患者生存预期较为乐观,但晚期患者预后堪忧.现行子宫内膜癌治疗方案包括外科切除、放射治疗、化学治疗及内分泌调控等手段,但针对进展期、播散性及复发患者,辅助治疗选择相对受限.作为一类免疫遗传学相关疾病,子宫内膜癌的发展进程深受肿瘤免疫微环境调控影响,这一观点已获众多研究证实.当前,分子层面的分类体系已被确立为子宫内膜癌分型的核心标准,但不同分子亚型子宫内膜癌所对应的肿瘤免疫微环境特性尚待阐明.本文系统梳理了各分子亚型子宫内膜癌免疫微环境特征的前沿研究成果,期望对临床诊疗子宫内膜癌有所帮助.
Endometrial cancer(EC)ranks among the three major gynecologic malignancies in women,with its incidence trending stages upward consistently on a global scale and the disease predominantly affecting postmenopausal women.Patients diagnosed at an early demonstrate relatively optimistic survival expectations,whereas those with advanced disease face concerning prognoses.Current therapeutic regimens encompass surgical resection,radiotherapy,chemotherapy,endocrine modulation and other modalities.For advanced,metastatic and recurrent cases,the options for adjuvant therapy are relatively limited.As an immunogenetic disease,the pathogenesis of endometrial cancer is profoundly regulated by the tumor immune microenvironment—a concept substantiated by numerous investigations.A finding that has been validated by numerous studies.At present,the molecular classification system has been established as the core criterion for EC subtyping,yet the characteristics of the tumor immune microenvironment corresponding to different molecular subtypes of EC remain to be elucidated.This paper systematically reviews the cutting-edge research findings on the tumor immune microenvironment characteristics of each molecular subtype of EC,with the aim of providing some assistance for clinical diagnosis and treatment of endometrial cancer.
