子宫内膜癌微创手术循证:从技术安全到精准决策OA

Endometrial cancer minimally invasive surgery:evolving evidence from surgical safety to precision decision-making

迄今,微创手术已用于子宫内膜癌治疗30余年.近年来机器人手术拓展了微创手术的临床应用范围,使微创手术在包括伴有高龄、病态肥胖等临床复杂应用场景中展现出良好的安全性与肿瘤学结局.然而,微创手术治疗子宫内膜癌亦面临诸多挑战.长期随访研究结果提示举宫器使用可能与子宫内膜癌肿瘤播散风险相关,肿瘤分子分型(如 TP53突变型、微卫星稳定型)可能影响手术预后.本文系统阐述子宫内膜癌微创手术治疗的发展历程、循证依据、临床隐患及未来发展方向,期待在确保肿瘤学安全基础上,结合患者病理、影像检查及分子分型特征进行评估及治疗决策,并加强临床各级医师规范培训以推动子宫内膜癌围手术期治疗决策从技术安全层面向精准个体化跨越.

Over the past three decades,minimally invasive surgery has been widely adopted in the treatment of endometrial cancer.Robotic-assisted surgery has expanded the clinical application scope of minimally invasive procedures,demonstrating favorable safety profiles and oncological outcomes even in complex clinical scenarios,including advanced age and morbid obesity.However,minimally invasive surgery also faces challenges.Long-term follow-up studies suggest that the use of uterine manipulators may be associated with a risk of tumor dissemination,while molecular subtypes of tumors—such as TP53 mutation and microsatellite stability(MSS)—may influence surgical prognosis.This article systematically reviews the developmental history,evidence-based foundations,clinical concerns,and future directions of minimally invasive surgical treatment for endometrial cancer.It is recommended that,on the basis of ensuring oncological safety,treatment decisions during the perioperative period should integrate patient pathology,imaging findings,and molecular subtype characteristics,Furthermore,standardized training for clinicians at all levels should be strengthened to facilitate the transition of perioperative treatment decision-making for endometrial cancer from technical safety toward precision-based individualized care.
