伤椎置钉联合椎弓根钉内固定术治疗胸腰椎骨折的效果OA
Effect of Screw Placement on the Injured Vertebrae Combined with Pedicle Screw Internal Fixation in the Treatment of Thoracolumbar Fractures
目的:探讨伤椎置钉联合椎弓根钉内固定术治疗胸腰椎骨折的效果.方法:选取2022年5月—2024 年 4 月仁怀市人民医院收治的 82 例胸腰椎骨折患者,以随机数字表法分成研究组与对照组,各41 例.对照组采用椎弓根钉内固定术治疗,研究组采用伤椎置钉联合椎弓根钉内固定术治疗.比较两组围手术期指标、胸腰椎功能、影像学指标及并发症发生情况.结果:两组住院时间、术中出血量、术后引流量及并发症发生率比较,差异无统计学意义(P>0.05);研究组手术时间长于对照组(P<0.05).术后 7 d、3 个月,两组改良Oswestry 功能障碍指数(ODI)评分低于术前,且研究组较对照组低(P<0.05);术后 7 d、3 个月,两组伤椎的中央高度、前缘高度、后缘高度均高于术前,伤椎后凸 Cobb 角小于术前,且研究组上述指标均优于对照组(P<0.05).结论:胸腰椎骨折患者采用伤椎置钉联合椎弓根钉内固定术治疗虽会延长手术时间,但能够改善胸腰椎功能,促进脊柱稳定性,改善伤椎影像学指标,且手术安全性较高.
Objective:To explore the effect of screw placement on the injured vertebrae combined with pedicle screw internal fixation in the treatment of thoracolumbar fractures.Method:A total of 82 patients with thoracolumbar fractures admitted to Renhuai People's Hospital from May 2022 to April 2024 were selected and divided into the study group(n=41)and the control group(n=41)by random number table method.The control group was treated with pedicle screw internal fixation,while the study group was treated with screw placement on the injured vertebrae combined with pedicle screw internal fixation.The perioperative indicators,thoracolumbar functions,imaging indicators and the occurrence of postoperative complications of the two groups were compared.Result:There were no statistically significant differences in the comparison of hospital stay,intraoperative blood loss,postoperative drainage volume and the incidence of complications between the two groups(P>0.05).The operation time of the study group was longer than that of the control group(P<0.05).At 7 days and 3 months after the operation,the modified Oswestry disability index(ODI)scores of both groups were lower than those before the operation,and the scores of the study group were lower than those of the control group(P<0.05).At 7 days and 3 months after the operation,the central height,anterior edge height and posterior edge height of the injured vertebrae in both groups were higher than those before the operation,and the kyphosis Cobb angle of the injured vertebrae was smaller than that before the operation,moreover,the above indicators in the study group were better than those in the control group(P<0.05).Conclusion:Although the treatment of thoracolumbar fractures with screw placement on the injured vertebrae combined with pedicle screw internal fixation may prolong the operation time,it can improve the function of the thoracolumbar vertebrae,promote spinal stability,improve the imaging indicators of the injured vertebrae,and has a relatively high surgical safety.
