深部热疗联合125I粒子植入治疗肝脏恶性肿瘤的近期效果与安全性OA
Short-term Efficacy and Safety of Deep Hyperthermia Combined with 125I Particle Implantation in the Treatment of Malignant Hepatic Tumor
目的:探讨深部热疗联合 125I 粒子植入治疗肝脏恶性肿瘤的近期效果与安全性.方法:选取 2023 年 11 月—2025 年 6 月复旦大学附属华东医院收治的肝脏恶性肿瘤患者 68 例,采用随机数字表法分为治疗组和对照组,每组 34 例.治疗组采用 125I 粒子植入联合深部热疗治疗,对照组仅采用 125I 粒子植入治疗.比较两组近期疗效、疼痛程度、生活质量及不良反应发生情况.结果:粒子植入后 3 个月,治疗组客观缓解率(ORR)为 85.3%(29/34),疾病控制率(DCR)为 100%(34/34),显著高于对照组的 61.8%(21/34)、82.4%(28/34)(P<0.05).粒子植入前,两组数字分级评分法(NRS)评分比较,差异无统计学意义(P>0.05);粒子植入后 0.5、1、1.5、2、2.5、3 个月,治疗组 NRS 评分低于对照组(P<0.05).粒子植入前,两组卡氏功能状态(KPS)评分比较,差异无统计学意义(P>0.05);粒子植入后 3 个月,两组 KPS 评分高于粒子植入前,且治疗组高于对照组(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:深部热疗联合 125I 粒子植入治疗肝脏恶性肿瘤的近期效果显著优于单纯 125I粒子植入治疗,能更有效地缓解患者疼痛,提高生活质量,且安全性相当.
Objective:To investigate the short-term efficacy and safety of deep hyperthermia combined with 125I particle implantation in the treatment of malignant hepatic tumor.Method:A total of 68 patients with malignant hepatic tumor admitted to Huadong Hospital Affiliated to Fudan University from November 2023 to June 2025 were selected.They were divided into a treatment group and a control group using a random number table method,with 34 cases in each group.The treatment group was treated with 125I particle implantation combined with deep hyperthermia,and the control group was treated with 125I particle implantation alone.The short-term efficacy,pain degree,quality of life,and the incidence of adverse reactions were compared between the two groups.Result:Three months after particle implantation,the objective response rate(ORR)in the treatment group was 85.3%(29/34),and the disease control rate(DCR)was 100%(34/34),which were significantly higher than 61.8%(21/34)and 82.4%(28/34)of the control group(P<0.05).Before particle implantation,there was no statistically significant difference in the numerical rating scale(NRS)scores between the two groups(P>0.05).At 0.5,1,1.5,2,2.5,and 3 months after particle implantation,the NRS scores of the treatment group were lower than those of the control group(P<0.05).Before particle implantation,there was no statistically significant difference in the Karnofsky performance status(KPS)scores between the two groups(P>0.05).Three months after particle implantation,the KPS scores of both groups were higher than those before implantation,and that in the treatment group was higher than the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The short-term effect of deep hyperthermia combined with 125I particle implantation in the treatment of malignant hepatic tumor is significantly better than that of 125I particle implantation alone.It can more effectively relieve patients'pain,improve their quality of life,and has comparable safety.
