清金化痰汤对慢性阻塞性肺疾病症状恢复及Nrf2/HO-1信号通路的影响OA
Effect of Qingjin Huatan Decoction on the Nrf2/HO-1 Signaling Pathway and Symptom Recovery in Chronic Obstructive Pulmonary Disease
目的:观察清金化痰汤对慢性阻塞性肺疾病(COPD)患者核因子E2相关因子2/血红素加氧酶-1(Nrf2/HO-1)及症状恢复的影响.方法:选择2023年9月至2025年5月就诊于我院的118例COPD患者为研究对象.通过简单化随机数字表法将其分为对照组和试验组,每组59例.对照组实施常规西医治疗,试验组采用清金化痰汤联合常规西医治疗,比较两组患者的证候积分、Nrf2与HO-1相对表达量、氧化应激、炎症指标及康复进程.分析Nrf2、HO-1相对表达量与临床指标的相关性.结果:治疗后,试验组咳黄痰多、胸闷气喘、发热口渴、烦躁不安等证候积分均短于对照组(P<0.05).试验组血清Nrf2、HO-1相对表达量均高于对照组(P<0.05).试验组超氧化物歧化酶(SOD)高于对照组,丙二醛(MDA)、白细胞计数(WBC)、中性粒细胞计数(Neu)均低于对照组(P<0.05).试验组呼吸困难缓解时间、咳嗽缓解时间、咳痰困难缓解时间、活动耐量恢复时间均低于对照组(P<0.05).Nrf2、HO-1相对表达量与SOD均呈正相关,与MDA、WBC、Neu均呈负相关(P<0.05).结论:清金化痰汤可缓解COPD患者临床证候,对减轻患者氧化应激、炎症反应并加快患者康复进程也有积极影响,其作用机制可能与激活Nrf2/HO-1通路有关.
Objective:To observe of the effect of Qingjin huatan decoction on Nrf2/HO-1 and symptom recovery in patients with chron-ic obstructive pulmonary disease(COPD).Methods:A prospective study was conducted on 118 COPD patients who visited our hospital from September 2023 to May 2025,and a parallel controlled trial was designed.They were divided into a control group and an experimental group using a simplified random number table method,with 59 cases in each group.The control group received conventional Western medicine treatment,while the experimental group received Qingjin Huatan decoction combined with conventional Western medi-cine.The syndrome scores,Nrf2/HO-1 signaling pathway proteins,oxidative stress,inflammatory indicators and rehabilitation process of the two groups of patients were compared.Analyze the correlation between Nrf2/HO-1 signaling pathway proteins and clinical indicators.Results:After treatment,the scores of symptoms such as cough with yellow phlegm,chest stuffiness and asthma,fever and thirst and restlessness in the experimental group were lower than those in the control group(P<0.05).The relative expression levels of serum Nrf2 and HO-1 in the experimental group were higher than those in the control group(P<0.05).The level of SOD in the experimental group was higher than that in the control group,while the MDA,WBC and Neu were lower than those in the control group(P<0.05).The relief time of dyspnea,cough,sputum and activity tolerance in the experimental group were lower than those in the control group(P<0.05).Nrf2 and HO-1 are positively correlated with SOD and negatively correlated with MDA,WBC and Neu(r=0.745,-0.802,-0.739,-0.779,0.795,-0.773,-0.725,-0.719;P<0.05).Conclusion:Qingjin huatan decoction can alleviate the clinical symptoms of COPD patients,and has a positive effect on reducing oxidative stress,inflammatory response and accelerating the recovery process of patients.Its mechanism of action may be related to the activation of the Nrf2/HO-1 pathway.
