目的:在数据库中筛选出含山药的处方,采用数据挖掘技术分析其配伍、主治病症及用量特征,总结山药的现代临床应用规律,旨在为山药在糖尿病肾病中的量化应用提供实证依据.方法:以"山药"为主题词,检索中国知网、万方、维普、PubMed数据库,筛选含山药的有效处方,运用IBM SPSS Modeler 18及SPSS Statistics 26统计软件,对含山药处方的配伍用药、主治病症及山药临床用量进行统计分析,并利用网络药理学技术分析山药在糖尿病肾病中的潜在作用.结果:共纳入现代含山药方剂804首,主治病症共150种,前5种高频疾病(≥25次)主要为消渴、泄泻、水肿、崩漏、眩晕,分别对应的证型为气阴两虚、脾胃虚弱、脾肾阳虚、肾阴亏虚、肾精不足.配伍药材药性主要为补气药、补阳药、补血药等;四气主要包括温、平、寒、凉等;五味药味主要为甘、苦、辛、淡等;主要归经为脾经、肾经、肺经、肝经等.治疗消渴时,山药的核心药对为"山药-山茱萸-丹参",常用剂量为15 g,功效偏向滋阴补肾、生津止渴;治疗水肿时,山药的核心药对为"山药-茯苓",常用剂量为15 g,山药偏向滋阴补肾、健脾消肿;治疗泄泻时,山药的核心药对为"山药-白术-甘草",常用剂量为10 g,山药偏向健脾益气止泄;治疗崩漏时,山药的核心药对为"山药-山茱萸-白芍",山药偏向滋阴养血止痛;治疗眩晕时,山药的核心药对为"山药-山茱萸",常用剂量为30 g,山药偏向滋阴补肾止眩.根据山药核心主治病症为消渴、水肿,二者与现代医学中的糖尿病肾病相对应.利用网络药理学技术进一步探讨了山药在治疗糖尿病肾病中的潜在活性成分和药理机制,提示山蒟酮、佛手酮、荜茇宁、胆固醇、薯蓣皂苷可能通过ERK/MAPK、JAK1/2、PI3K/Art等通路靶向PIK3CA、SRC、PIK3CD、PIK3CB、PTPN11等靶点,减轻炎症反应、调节胰岛素抵抗等,发挥潜在的治疗作用.结论:本研究通过数据挖掘对山药的现代应用进行了系统总结,并利用网络药理学技术揭示了山药在治疗糖尿病肾病的潜在机制,可为山药的临床应用提供依据.

Objective:This study systematically screened Chinese herbal prescriptions containing Shanyao(Rhizoma Dioscoreae)from Chinese and English literature databases and utilized data mining techniques to uncover its medication rules,aiming to provide references and evidence for the clinical application of Shanyao,especially its precise application in diabetic kidney disease.Methods:Using"Shan-yao"as the subject term,Chinese and English literature databases including CNKI,Wanfang,VIP,and PubMed were searched to screen out effective prescriptions containing Shanyao.Statistical software IBM SPSS Modeler 18 and SPSS Statistics 26 were employed to analyze the compatibility of medicines,indications and clinical dosage of Shanyao in the included prescriptions.Network pharmacology technology was further applied to analyze the potential role of Shanyao in the treatment of diabetic kidney disease.Results:A total of 804 modern prescriptions containing Shanyao were included,covering 150 indications.The top five high-frequency diseases(≥25 occurrences)were primarily Wasting-Thirst,Diarrhea,Edema,Uterine Bleeding and Vertigo.The corresponding syndrome patterns were Qi and Yin Defi-ciency,Spleen-Stomach Deficiency,Spleen-Kidney Yang Deficiency,Kidney Yin Deficiency and Kidney Essence Deficiency,respective-ly.The compatible herbs were primarily Qi-tonifying medicines,Yang-tonifying medicines and Blood-tonifying medicines.The four natures were mainly Warm,Neutral,Cold and Cool.The five flavors were mainly Sweet,Bitter,Pungent,and Bland.The primary meridian tropism were the Spleen,Kidney,Lung and Liver meridians.For treating Wasting-Thirst,the core herb pairs for Shanyao were"Shanyao-Shanzhuyu(Fructus Corni)-Danshen(Salviae Miltiorrhizae)",with a common dosage of 15 g for Shanyao,emphasizing its effect in nourishing Yin,tonifying the Kidney,promoting fluid production,and quenching thirst.For Edema,the core herb pair was"Shanyao-Fuling(Poria)",with a common dosage of 15 g for Shanyao,highlighting its role in nourishing Yin,tonifying the Kidney,strengthening the Spleen,and allenating edema.For Diarrhea,the core herb pairs were"Shanyao-Baizhu(Rhizoma Atractylodis Macro-cephalae)-Gancao(Radix et Rhizoma Glycyrrhizae)",with a common dosage of 10 g for Shanyao,focusing on its action in strengthening the Spleen,boosting Qi,and stop diarrhea.For Uterine Bleeding,the core herb pairs were"Shanyao-Shanzhuyu(Fructus Corni)-Baishao(Radix Paeoniae Alba)",emphasizing Shanyao's function in nourishing Yin,nourishing Blood,and relieving pain.For Vertigo,the core herb pair was"Shanyao-Shanzhuyu(Fructus Corni)",with a common dosage of 30 g for Shanyao,underscoring its effect in nourishing Yin,tonifying the Kidney,and relieving vertigo.Using network pharmacology,the potential active components and pharmaco-logical mechanisms of Shanyao in treating diabetic kidney disease were further explored.It suggested that compounds such as Hancinol,Kadsurenone,Piperlonguminine,Cholesterol,and Diosgenin might target key targets like PIK3CA,SRC,PIK3CD,PIK3CB,and PTPN11 through pathways like ERK/MAPK,JAK1/2,and PI3K/Akt,thereby potentially alleviating inflammatory responses,regulating insulin resistance,and exerting therapeutic effects.Conclusion:Through data mining,this study systematically summarizes the modern application of Shanyao.Furthermore,network pharmacology reveals the potential mechanisms of Shanyao in treating diabetic kidney disease,which can provide references for its clinical promotion.
