代谢功能障碍相关脂肪性肝病(MASLD)及其进展形成的代谢功能障碍相关脂肪性肝炎(MASH)是全球范围内面临的严峻健康挑战,其特点是普遍流行、自然进展及致病机制复杂.近年来MASLD患病率逐年上升,尤其是向终末期肝病转化严重影响患者预后.虽然针对该适应证美国FDA已批准了2种药物(瑞美替罗和司美格鲁肽),但现有疗法远未满足临床需求,仍不足以改善MASLD晚期纤维化等病征,以及预防重要并发症发生,尤其是其临床疗效差异性及个性化治疗仍待解决.MASLD多维交织的复杂病理机制和异质性使单一靶点药物难以实现全面覆盖,迫切需要研发具有新机制、新靶点的药物,以及新型干预治疗手段.因此,本文综述MASLD/MASH的致病机制、干预靶点及治疗药物(包括化学治疗药物、细胞治疗药物和基因治疗药物)的研究进展,重点聚焦化学和细胞治疗药物,分析了现有化学药物和干细胞及其外泌体治疗存在的问题和面临的挑战;阐明了不同靶点药物联合应用或开发多靶点药物可能是今后MASLD/MASH治疗或新药研发的方向.干细胞及外泌体作为MASLD/MASH的新型干预手段或临床治疗补充展示了良好前景,但其异质性、稳定性、长期安全性及疗效可靠性仍需更多临床研究验证.

Metabolic dysfunction-associated steatotic liver disease(MASLD)and its progressive condition-metabolic dysfunction-associated steatohepatitis(MASH)-are posing a significant challenge to human health worldwide.This disease is characterized by high prevalence,natural progression,and complex pathogenic mechanisms.In recent years,the incidence of MASLD has been increasing.What's worse,its progression to end-stage liver disease can impact the patient's prognosis.Despite the two drugs(resmetirom and semaglutide)approved by the U.S.Food and Drug Administration(FDA)for this indication,therapies currently available fail to meet real-world clinical needs,ameliorate such symptoms as the advanced liver fibrosis in MASLD or to prevent the occurrence of major compli-cations.In particular,the personalized treatments for the MASLD with different pathological characteris-tics at different stages need to be made available.The heterogeneity and complex pathological process involving intertwined multiple mechanisms of MASLD make single-target drugs ineffective for all related cases,pointing to a pressing need to develop drugs with new mechanisms and new targets and to find new intervention methods.This paper reviews the progress in the pathogenic mechanisms,intervention targets,and therapeutic drugs(including chemotherapeutic drugs,cell therapy drugs and gene therapy drugs)concerning MASLD/MASH in general and chemotherapeutic drugs and stem cell-based thera-peutic drugs in particular.This review also analyzes the issues and challenges facing chemical drugs and stem cells and their exosomes for treatment of MASLD/MASH,and asserts that the combination of different single-target drugs or the development of multi-target drugs may be the priority of research on MASLD/MASH.Mesenchymal stem cells and their exosomes show good prospects as a novel means of intervention or a supplement to clinical treatment for MASLD/MASH.However,its heterogeneity,sta-bility,long-term safety,and reliability of efficacy need to be verified.
