首发未吸烟精神分裂患者外周犬尿氨酸通路的特点OACHSSCD
Peripheral Kynurenine metabolic pathway in no-smoking patients with first-episode schizophrenia
目的:探究首发未吸烟精神分裂症(FENS)患者外周血犬尿氨酸代谢通路(KP)中的神经活性产物浓度及其与精神病理症状间的关系.方法:纳入 82 例 FENS 患者和 73 例正常对照,检测两组血清 KP 关键代谢物水平,采用阳性和阴性症状量表(PANSS)评估患者症状,分析代谢物浓度与症状评分的相关性.结果:FENS 组血清犬尿氨酸(KYN,251.5±65.9 ng/mL)、犬尿喹啉酸(KYNA,5.2±2.2 ng/mL)、喹啉酸(QUIN,39.5±13.0 ng/mL)浓度均低于正常对照组(KYN,320.7±65.9 ng/mL,KYNA,13.3±4.2 ng/ml,QUIN,58.8±16.9 ng/mL)(t=-6.38、-14.73、-7.67,均 P<0.001).FENS 患者血清 KYNA 浓度与 PANSS 一般病理量表评分呈正相关(r=0.30,P=0.007).结论:FENS 患者存在 KP 神经活性代谢物异常,且与精神病理症状相关,提示 KP 代谢紊乱可能参与精神分裂症的病理生理过程.
Objective:To investigate peripheral alterations of neuroactive metabolites in kynurenine pathway(KP)and their associations with psychiatric symptoms in non-smoking patients with first episode schizophrenia(FENS).Methods:Eighty-two FENS and 73 normal controls were recruited.Serum concentrations of kynurenine(KYN),kynurenic acid(KYNA),and quinolinic acid(QUIN)were measured using a combination of liquid chro-matography with tandem mass spectrometry.The Positive and Negative Syndrome Scale(PANSS)was used to as-sess Psychopathology.The t-test and analysis of covariance(ANCOVA)was used to comparison serum levels of product of Kynurenine metabolic pathway between patients with FENS and normal controls.The χ2 test was per-formed to examine differences in categorical variables between the two groups.Pearson's correlations were used to test the associations between variables in two groups respectively.Results:Serum levels of KYN(251.5±65.9 ng/mL),KYNA(5.2±2.2 ng/mL),QUIN(39.5±13.0 ng/mL)in FENS were significantly lower than those in normal controls(KYN,320.7±65.9 ng/mL;KYNA,13.3±4.2 ng/mL;QUIN,58.8±16.9 ng/mL)(t=-6.38,-14.73,-7.67,Ps<0.001).In FENS patients,Serum KYNA of FENS patients was positively correlated with the general pathological scale of PANSS scale(r=0.30,P<0.01).Conclusion:FENS patients show signifi-cant abnormalities in peripheral KP metabolites,which are associated with psychiatric symptoms.These findings suggest that KP dysregulation may contribute to the pathophysiology of schizophrenia.
