抑郁症脑前额叶与颞叶激活特征及其与临床症状和认知功能的关系OACHSSCD
Prefrontal and temporal hemodynamic characteristics of patients with major depressive disorder and their associations with clinical symptoms and cognitive function
目的:分析抑郁症(MDD)患者认知功能的改变以及 MDD 患者在言语流畅性任务(VFT)期间额颞叶血流动力学特征,探究其与抑郁症状、焦虑症状及认知功能障碍严重程度的相关性.方法:招募40 例 MDD 患者与 40 例年龄、性别及受教育程度相匹配的正常对照组,采用汉密顿抑郁量表(HAMD)、贝克抑郁量表(BDI)、汉密顿焦虑量表(HAMA)及贝克焦虑量表(BAI)评估 MDD 患者临床症状的严重程度.使用数字符号转换测验(DSST)、数字广度测验(DST)、语义言语流畅性任务(SVF)和音韵言语流畅性任务(PVF)评估受试者的认知功能.使用近红外光谱成像仪监测被试在 VFT期间前额叶及双侧颞叶血流动力学特征.结果:MDD 组的 DSST 成绩(P<0.001)、PVF 总成绩(P<0.001)及前额叶积分值(P<0.05)均低于正常对照组.MDD 组前额叶重心值与 HAMD 阻滞因子分呈正相关(r=0.33,P<0.05),双侧颞叶积分值与 HAMD 绝望感因子得分呈负相关(r=-0.35,P<0.05).结论:MDD 患者存在一定程度的认知功能受损,且其前额叶与颞叶血流动力学特征与其临床症状严重程度具有一定相关性.
Objective:To investigate cognitive function of patients with major depressive disorder(MDD),ex-amine frontotemporal lobe hemodynamic responses during the verbal fluency text(VFT)task in patients with MDD,and explore their correlation with depressive symptoms,anxiety symptoms,and severity of cognitive dysfunc-tion.Methods:Forty MDD patients and 40 normal controls matched by age,gender and years of education were en-rolled.The Hamilton Depression Scale(HAMD),Beck Depression Inventory(BDI),Hamilton Anxiety Scale(HA-MA)and Beck Anxiety Inventory(BAI)were used to evaluate the severity of clinical symptoms.The cognitive function of all subjects was evaluated by Digit Symbol Substitution Test(DSST),Digita Span Test(DST),Seman-tic Verbal Fluency task(SVF)and Phonological Verbal Fluency task(PVF).Subjects were monitored for prefrontal and bilateral temporal lobe hemodynamic characteristics during the VFT using functional near-infrared spectroscopy.Results:Compared with controls,the DSST scores(P<0.001),PVF total scores(P<0.001),and prefrontal lobe integral values(P<0.05)were lower in the MDD group than in the normal control group.The pre-frontal center of gravity values in the MDD group were positively correlated with the HAMD retardation factor scores(r=0.33,P<0.05),while the bilateral temporal lobe integral values were negatively correlated with the HAMD hopelessness factor scores(r=-0.35,P<0.05).Conclusion:Patients with MDD exhibit notable cogni-tive impairment,and the hemodynamic characteristics of their frontal and temporal lobes are correlated with the se-verity of their clinical symptoms.
