基于血红蛋白、白蛋白、淋巴细胞和血小板的营养评分系统对多发性骨髓瘤患者预后的影响研究OA
Effect of nutritional scoring system based on hemoglobin,albumin,lymphocytes and platelets on prognosis of patients with multiple myeloma
目的 分析由血红蛋白、白蛋白、淋巴细胞和血小板组成的营养评分(HALP评分)在多发性骨髓瘤(MM)患者预后评价中的价值.方法 回顾性分析2020年12月至2024年12月天津医科大学第二医院血液科收治的107例初诊MM患者的临床资料并进行随访,通过患者外周血中的血红蛋白含量、白蛋白含量、淋巴细胞计数和血小板计数计算HALP评分.采用ROC曲线分析HALP评分的最佳截断值,Kaplan-Meier法绘制生存曲线,Log-rank检验和Cox回归分析影响患者预后的可能因素.结果 通过ROC曲线计算出HALP评分最佳截断值(20.8分),将患者分为两组[低评分组:HALP<20.8分(n=49);高评分组:HALP≥20.8分(n=58)].与高评分组相比,低评分组的总生存时间(OS)和无进展生存时间(PFS)更差(P=0.003,P=0.005).单因素分析显示年龄>60岁、β 2-微球蛋白(β2-MG)>5.0 mg/L、ISS分期Ⅲ期和低HALP组(HALP<20.8分)是影响多发性骨髓瘤患者OS的危险因素(P=0.00 1,P=0.031,P=0.021,P=0.003).多因素Cox回归分析显示:年龄>60岁和低HALP组(HALP<20.8分)和ISS分期Ⅲ期是影响MM患者OS的独立危险因素(P=0.005,P=0.008,P=0.021).结论 HALP评分对多发性骨髓瘤患者的预后有一定的预测价值.
Objective To analyze the prognostic value of the nutritional score composed of hemoglobin,albumin,lymphocytes,and platelets(HALP score)in patients with multiple myeloma(MM).Methods The clinical data of 107 newly diagnosed MM patients in the Second Hospital of Tianjin Medical University from December 2020 to December 2024 were collected.The HALP score was calculated based on the levels of hemoglobin,albumin,lymphocyte count,and platelet count in peripheral blood.ROC curve analysis was used to calculate the optimal cutoff value of HALP scores.Kaplan-Meier method was used to draw survival curves.Log-rank test and COX regression analysis were used to explore possible factors affecting patient prognosis.Results The optimal cut-off value with the ROC curves for the HALP score was 20.8.The patients were divided into two groups according to the optimal value of the HALP score[Low-score group:HALP<20.8(n=49)and high-score group HALP≥20.8(n=58)].The OS and PFS of the high HALP score group were higher than those of the low HALP score group(P=0.003,P=0.005).Univariate analysis showed thatage>60,β 2-microglobulin>5.0 mg/L,ISS stage(Ⅲ),and low HALP score(HALP<20.8)were prognostic risk factors for OS in multiple myeloma patients(P=0.001,P=0.031,P=0.021,P=0.003).Multivariate Cox regression analysis revealed that age>60,low HALP score(HALP<20.8)and ISS stage(Ⅲ)were independent risk factors for OS in MM patients(P=0.005,P=0.008,P=0.021).Conclusion The H ALP score may be a valuable prognostic indicator in multiple myelomapatients.
