宫缩量化技术与安全分娩:从基础研究到临床应用OA
Uterine contraction quantification technology and safe delivery:from basic research to clinical application
有效、安全的子宫收缩是分娩的核心动力,而传统宫缩评估方法的局限性,制约了产程产力的精准管理.无创子宫肌电图(uEMG)技术通过实时捕捉子宫平滑肌电活动特征,能够无创反映宫缩起始、传播方向与强度分布,可实现宫缩频率、持续时间及同步性等参数的自动提取,实现宫缩监测量化.通过子宫生物电活动的基础生理进一步阐述了无创uEMG技术发展,其在产程中宫缩力客观监测与异常诊断、早产精准预测、高危妊娠个体化分娩时机决策等方面,均展示了精细化评估的临床应用价值,为宫缩力临床干预提供了全新视角.未来需进一步推动技术标准化及人工智能化,促进其与胎心监护等多模态信息的整合,以构建智能化的安全产程管理体系以实现安全分娩.
Effective and safe uterine contractions are the core driving force of childbirth,and the limitations of tradi-tional assessment methods of uterine contractions have re-stricted the precise management of labor progress and uter-ine contractility.Non-invasive uterine electromyography(uEMG)technology can non-invasively reflect the onset,propagation direction,and intensity distribution of uterine contractions by capturing the electrical activity characteris-tics of uterine smooth muscle in real time.It can automati-cally extract parameters such as uterine contraction fre-quency,duration,and synchrony,thereby realizing the quantification of uterine contraction monitoring.The devel-opment of non-invasive uEMG technology is further elabo-rated based on the basic physiology of uterine bioelectrical activity.It has demonstrated the clinical application value of refined assessment in aspects such as the objective moni-toring and abnormal diagnosis of uterine contractility dur-ing labor,the accurate prediction of preterm birth,and the individualized decision-making of delivery timing in high-risk pregnancies,providing a new perspective for the clini-cal intervention of uterine contractility.In the future,it is necessary to further promote the standardization and AI in-telligence of the technology,and facilitate its integration with multi-modal information such as fetal heart rate moni-toring,so as to construct an intelligent safe labor manage-ment system and achieve safe childbirth.
