口腔样本及其分离的念珠菌中幽门螺杆菌特异性基因的检测OA
Detection of Helicobacter pylori-specific genes in oral samples and isolated Candida
目的 通过对消化内科就诊患者口腔样本进行幽门螺杆菌(Helicobacter Pylori,Hp)特异性核酸检测,分析口腔Hp检出情况,并对口腔样本分离的念珠菌中是否存在Hp特异性基因进行研究.方法 采集200例行 13C尿素呼气试验(13C-urea breath test,13C-UBT)检查Hp的消化内科患者的口腔样本,进行念珠菌的分离,检测口腔样本和念珠菌样本中的Hp特异性核酸,以明确口腔样本及其分离的念珠菌样本中Hp特异性基因检出情况.结果 200例患者中 13C尿素呼气试验阳性和阴性分别为93例、107例;口腔样本Hp特异性16S rDNA基因阳性率为22.83%,ureA基因阳性率为20.33%,16S rDNA、ureA双阳性的阳性率为9.33%;口腔不同部位Hp检出率差异无统计学意义(χ2=1.930,P>0.05);胃Hp阳性组口腔Hp检出率高于胃Hp阴性组(χ2=20.480,P<0.05).口腔样本念珠菌检出率为10.5%,以白念珠菌检出最高;口腔样本分离念珠菌经传代培养至第5代的样本中Hp特异性16S rDNA阳性率为39.68%,ureA阳性率为52.38%,16S rDNA、ureA双阳性的阳性率为14.29%.结论 口腔样本和口腔念珠菌均可检测到Hp特异性基因;胃Hp感染增加了口腔Hp特异性基因的检出率.
This study analyzed oral detection of Helicobacter pylori(Hp)through Hp-specific nucleic acid testing on oral samples from patients at the Department of Gastroenterology at our institution.The presence of Hp-specific genes in Candida isolated from oral samples was investigated.Oral samples of 200 gastroenterology patients who underwent 13C-urea breath testing(13C-UBT)for Hp were collected and cultured for Candida isolation.Hp-specific nucleic acids in the oral samples and Candida samples were de-tected.Of the 200 included patients,93 and 107 had positive and negative 13C urea breath test results,respectively.The positivity rates were 22.83 for the Hp-specific 16S rDNA gene in oral samples and 20.33%for the ureA gene.The rate of 16S rDNA and ureA double positivity was 9.33%.The detection rates of Hp in different parts of the oral cavity showed no statistically significant differences(χ2=1.930,P>0.05).The detection rate of oral Hp was significantly higher in the gastric Hp-positive group than the gastric Hp-negative group(χ2=20.480,P<0.05).The detection rate of Candida in oral samples was 10.5%,and Candida albicans showed the high-est rate among species.The positivity rate of Hp-specific 16S rDNA in samples isolated from Candida in oral samples cultured by pas-saging to the 5th generation was 39.68%;that of ureA was 52.38%;and that of both Hp 16S rDNA and ureA was 14.29%.Therefore,Hp-specific genes were detected in both oral samples and oral Candida,and gastric H.pylori infection increased the detection rate of oral H.pylori-specific genes.
