Objective To explore effective organizational forms for hierarchical diagnosis and treatment and chronic disease management,providing replicable reference experiences for the sustainable development of county-level integrated healthcare systems.Methods Based on the analysis of work experience during the pilot phase of the integrated healthcare system,combined with field research and expert discussions to address existing issues,practical initiatives were implemented at the regional grassroots level to explore the construction of a new integrated healthcare system model.Results After five years of practice,an integrated prevention-treatment sys-tem was proposed and established,focusing on general-specialist joint clinics and chronic disease joint wards.This system addressed deep-seated institutional and mechanistic contradictions,broke the bottleneck of limited high-quality medical resources at the grassroots level,enhanced primary care service capabilities,and explored a practi-cal and sustainable reform path to achieve the three enhancements(enhancing grassroots primary care,enhancing resource allocation to grassroots levels,and enhancing chronic disease management).Conclusions General-spe-cialist joint clinics and chronic disease joint wards are effective organizational forms for hierarchical diagnosis and treatment and chronic disease management,providing practical references for the reform of county-level integrated healthcare systems.
