DRG付费全球应用情况及实施效果的经验和启示OA

Global implementation and impact of DRG payment:Experiences and insights

DRG付费是我国正在加快推进的支付制度改革.文章检索并系统分析了全球范围内DRG付费的应用情况,重点关注OECD成员国和亚洲国家的实施效果,总结了其实施规律与经验.研究发现,OECD成员国从引入DRG到全面推广应用,普遍经历了较长的实验探索阶段,期间克服了高质量数据保障、分组方案动态更新等技术难题,以及来自利益相关者的阻力.目前,38个OECD成员国中的34个选择了 DRG付费,通过提升效率、控制费用并强化卫生治理能力,实现了显著的制度性收益.基于国际经验,文章提出引导医院正确看待DRG付费、保障病案数据质量、优化医疗质量监管体系等建议,同时面对区域点数法等独特制度安排所带来的新问题,医保改革仍需持续关注与应对.

Diagnosis-Related Group(DRG)-based payment is a is a payment system reform being accel-erated in China.This study systematically reviews the global implementation of DRG payment systems,with a par-ticular focus on OECD member countries and Asian nations.It summarizes the underlying patterns and practical experiences.Findings indicate that OECD countries typically underwent extended pilot phases before full-scale a-doption of DRG systems.During these phases,they addressed significant technical challenges such as ensuring high-quality clinical data and maintaining dynamic group classification systems,while also overcoming resistance from key stakeholders.Ultimately,34 out of 38 OECD member countries adopted DRG-based payment,achieving substantial systemic benefits including enhanced healthcare efficiency,cost containment and strengthened govern-ance capabilities.Drawing on international experience,this paper proposes several policy recommendations,inclu-ding fostering a correct understanding of DRG mechanisms among hospitals,enhancing the quality of medical re-cord data,and optimizing medical quality supervision frameworks.Furthermore,the study underscores the need for continuous attention to emerging challenges,such as those posed by region-specific mechanisms like the regional point-scoring method,within the broader context of ongoing medical insurance reform.
