目的 探讨动态增强磁共振成像(DCE-MRI)在肥大乳房乳头乳晕复合体(NAC)血供评估中的价值.方法 回顾性分析2024年1月至2025年1月我院收治的107例肥大乳房患者(观察组)和同期100例正常乳房志愿者(对照组)的DCE-MRI影像资料,通过最大密度投影(MIP)图像识别NAC供血血管,并按9分区法统计血供来源.结果 观察组患者乳房单区供血率显著高于对照组(P<0.05),双区供血为2组患者乳房最常见的供血模式,2组患者双区、3区及以上供血率比较,差异均无统计学意义(P>0.05).2组患者乳房NAC血供区域分布比较,差异均无统计学意义(P>0.05).2组患者乳房血供区域均主要集中于内上象限和外上象限.结论 DCE-MRI可清晰地显示肥大乳房的NAC血供特征,肥大乳房的NAC血供模式与正常乳房相似,但其单区供血比例显著高于正常乳房.术前DCE-MRI评估有助于指导手术切口设计及血管保护,降低术后NAC坏死风险.

Objective To explore the value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in assessing blood supply to the nipple-areola complex(NAC)in breast hypertrophy.Methods The DCE-MRI data of 107 patients with breast hypertrophy(the observation group)and 100 volunteers with normal breast(the control group)admitted to our hospital from January 2024 to January 2025 were retrospectively analyzed.The blood supply vessels to NAC were identified by maximum intensity projection(MIP)images,and the blood supply sources were counted according to the 9-region method.Results The single-region blood supply rate of the breasts in the observation group was significantly higher than that in the control group(P<0.05).The dual-region blood supply was the most common blood supply pattern for the breasts in both groups.No statistically significant differences were found between the two groups in the rates of dual-zone or three-or-more-region blood supply(P>0.05).There was no statistically significant difference in the regional distribution of NAC blood supply in the breasts between the two groups(P>0.05).The blood supply regions of the breasts in both groups were mainly concentrated in the inner upper quadrant and the outer upper quadrant.Conclusion DCE-MRI can clearly display the NAC blood supply characteristics of breast hypertrophy.The NAC blood supply pattern in breast hypertrophy is similar to that in normal breasts,while the proportion of single-region blood supply is significantly higher than that of normal breasthan.Preoperative assessment of DCE-MRI can help guide the design of surgical incisions and vascular protection,thus reducing the risk of NAC necrosis after surgery.
