目的 探讨采用对比增强定量磁共振成像(qMRI)技术评估人前交叉韧带(ACL)不同区域灌注特征的临床应用价值.方法 回顾性分析42例患者膝关节的对比增强qMRI检查资料,根据矢状斜面增强前图像,将ACL全长等分为近端、中部和远端三个区域(各占1/3长度),通过量化MRI信号增强程度,计算各区域灌注量占ACL总灌注量的相对百分比,并分析其在男女间的差异.结果 ACL近端、中部和远端区域的相对灌注量比较,差异具有统计学意义(P<0.05);其中近端区域相对灌注量显著高于中部(P=0.007)和远端区域(P=0.001),中部与远端区域间相对灌注量比较差异无统计学意义(P=0.281).亚组分析显示,男性ACL近端区域灌注量高于女性,差异具有统计学意义(P=0.036).结论 本研究通过对比增强qMRI技术证实,ACL血管分布主要集中于近端1/3区域,占总灌注量的一半以上.明确ACL血管分布的区域差异,可为理解ACL 损伤的发生机制、优化ACL 损伤修复重建方案提供重要的影像学理论依据.

Objective To explore the clinical application value of contrast-enhanced quantitative magnetic resonance imaging(qMRI)in evaluating the perfusion characteristics in different regions of the human anterior cruciate ligament(ACL).Methods The contrast-enhanced qMRI data of the knee joints in 42 patients were retrospectively analyzed.According to the pre-contrast oblique sagittal images,the entire length of the ACL was equally divided into three regions:proximal,middle and distal regions(each accounting for 1/3 of the total length).The relative percentage of perfusion in each region to the total ACL perfusion was calculated by quantifying the degree of MRI signal enhancement.Results There were statistically significant differences in the relative perfusion among the proximal,middle and distal regions of the ACL(P<0.05).The relative perfusion of the proximal region was significantly higher than that of the middle(P=0.007)and distal regions(P=0.001),while there was no statistically significant difference in relative perfusion between the middle and distal regions(P=0.281).Subgroup analysis showed that the relative perfusion of the proximal region of the ACL in males was higher than that in females,with a statistically significant difference(P=0.036).Conclusion Based on contrast-enhanced qMRI,this study confirmed that the vascular distribution of the ACL is mainly located in the proximal 1/3 region,accounting for more than half of the total perfusion.Clarifying the regional differences of ACL vascular distribution may provide an important imaging theoretical basis for understanding the pathogenesis of ACL injury and optimizing surgical strategies for ACL repair and reconstruction.
