胃癌潜在新型生物标志物临床探索研究进展与展望OA

Progress in clinical exploration of potential novel biomarkers in gastric cancer

胃癌是全球高发的恶性肿瘤,早期诊断困难、治疗反应个体差异大、复发监测手段有限及预后评估准确性不足等问题长期制约患者生存率提升.随着多组学技术和液体活检的发展,胃癌生物标志物研究进入精准化、动态化新阶段.本文系统梳理了胃癌的早期筛查、疗效预测、复发监测及预后评估4个关键领域的研究进展.其中,在早期筛查方面,循环核酸与代谢标志物提升了无创检测性能;在疗效预测方面,新型标志物为个体化治疗提供依据;在复发监测方面,分子影像学实现了对微小残留病灶的精准捕捉;在预后评估方面,多组学整合鉴定出多个具有独立预后价值的分子标志物.当前研究呈现整合化趋势,但仍面临标志物异质性、临床转化效率低、检测标准化不足等挑战.未来需建立分层级标志物诊断体系,推动前瞻性和干预性临床试验,通过关注特殊的胃癌亚型,开发可及性检测手段,通过与新兴治疗的方式和新的治疗药物的使用相结合,推动生物标志物从研究发现向临床实践转化,最终改善胃癌患者的预后.

Gastric cancer is a highly prevalent malignancy worldwide.Challenges including difficult early diagnosis,substantial inter individual variability in treatment response,limited modalities for recurrence surveillance,and insufficient accuracy of prognostic assessment have long constrained improvements in patient survival.With advances in multi-omics technologies and liquid biopsy,gastric cancer biomarker research has entered a new era of precision and dynamic monitoring.This review systematically summarizes research progress across four critical domains:early screening,therapeutic efficacy prediction,recurrence surveillance,and prognostic evaluation.Circulating nucleic acids and metabolic biomarkers have enhanced the performance of noninvasive detection for early screening.Novel biomarkers provide evidence for individualized therapy in efficacy prediction.Molecular imaging enables precise detection of minimal residual disease in recurrence surveillance.Multi-omics integration has identified multiple molecular biomarkers with independent prognostic value.Current research demonstrates a trend toward integration,yet challenges persist,including biomarker heterogeneity,inefficient clinical translation,and inadequate standardization of detection methods.Future efforts should focus on establishing hierarchical biomarker systems,advancing prospective validation and interventional trials,developing accessible detection platforms,addressing special histological subtypes,and exploring combinatorial applications with emerging therapeutics to facilitate the translation of biomarkers from research discoveries to clinical practice,ultimately improving outcomes for gastric cancer patients.
