Clinical implications of tumour and stromal tissues immune cell infiltration in prognostication and immunotherapy response of gastric cancer:a retrospective cohort study

背景和目的:现有研究多聚焦于单一免疫细胞类型,缺乏对肿瘤/间质组织中整体免疫细胞浸润谱的系统分析.本研究旨在探索免疫细胞浸润区域异质性的临床意义.方法:本研究回顾性收集了2004年3月—2009年7月于南通市第三人民医院就诊的符合本研究纳入及排除标准的胃癌患者的临床标本以及相应的临床信息,利用免疫细胞表面标志物标记组织中8种免疫细胞(CD3+细胞、CD3+CD4+细胞、CD3+CD8+细胞、CD3+FOXP3+细胞、CD68+细胞、CD66b+细胞、CD11b+细胞和CD45RO+细胞),分别分析了免疫细胞在肿瘤和间质中的浸润水平以及临床意义.本回顾性队列研究获得南通市第三人民医院伦理审查委员会批准(审查编号:EK2025062)并获得患者的知情同意.结果:最终有405例符合入组标准的患者纳入本研究.肿瘤组织中CD68+细胞和间质中CD68+细胞的浸润水平在不同肿瘤分期、分级以及组织分化程度上差异有统计学意义(P<0.05),无论间质中的 CD68+细胞(HR=0.971,95%CI:0.960~0.982,P<0.001)还是肿瘤中的CD68+细胞(HR=1.012,95%CI:1.006~1.018,P<0.001)都可以很好地预测胃癌患者的生存率,是胃癌患者预后的独立影响因素.间质中CD68+细胞的浸润水平和免疫检查点分子细胞毒性T淋巴细胞相关抗原4(cytotoxic T-lymphocyte-associated antigen 4,CTLA-4)、程序性死亡受体 1(programmed cell death-1,PD-1)、程序性死亡配体1(programmed cell death-ligand 1,PD-L1)和淋巴细胞活化基因3(lymphocyte-activation gene 3,LAG3)的表达水平均显著相关(P<0.05).结论:间质中CD68+细胞高浸润水平、肿瘤组织中CD68+细胞低浸润水平都具有良好的预后预测价值,本研究结果为后续CD68⁺细胞相关研究指明了区域特异性的方向.

Background and purpose:Most existing studies have focused on single immune cell types,lacking systematic analyses of the global immune cell infiltration landscape within tumor parenchyma and stroma.This study aimed to investigate the clinical significance of regional heterogeneity in immune cell infiltration.Methods:This study retrospectively collected clinical specimens and the corresponding clinical information from gastric cancer patients who were treated in Nantong Third People's Hospital from March 2004 to July 2009 and met the inclusion and exclusion criteria of this study.This study used immune cell surface markers to identify 8 types of immune cells in the tissues(CD3+cells,CD3+CD4+cells,CD3+CD8+cells,CD3+FOXP3+cells,CD68+cells,CD66b+cells,CD11b+cells,and CD45RO+cells).We separately analyzed the expression levels of these immune cells in tumor and stromal compartments,as well as their clinical significance.This retrospective cohort study was approved by the Ethics Committee of Nantong Third People's Hospital(approval No.EK2025062)and the informed consents of the patients were acquired.Results:A total of 405 patients who met the inclusion criteria were finally included in this study.The infiltration level of CD68+cells showed significant differences across different tumor stages,grades,and histological differentiation statuses(P<0.05).Both stromal CD68⁺ cells[hazard ratio(HR)=0.971,95%CI:0.960-0.982,P<0.001]and tumor CD68⁺ cells(HR=1.012,95%CI:1.006-1.018,P<0.001)exhibited robust predictive efficacy for patient survival outcomes,and were identified as independent prognostic factors for gastric cancer.Moreover,stromal CD68 ⁺ cell infiltration levels were significantly correlated with the expression of immune checkpoints,including cytotoxic T-lymphocyte-associated antigen 4(CTLA-4),programmed cell death-1(PD-1),programmed cell death-ligand 1(PD-L1)and lymphocyte-activation gene 3(LAG3,P<0.05).Conclusions:High infiltration of CD68⁺ cells in the stromal compartment and low infiltration of CD68⁺ cells in the tumor parenchyma conferred favorable prognostic value for gastric cancer patients.This study provided a region-specific framework for future investigations targeting CD68⁺ cells.
