基于证素辨证原理的"症状-证素"诊断权值归一化的研究与思考OA
Research and Reflection on Normalization of Diagnostic Weights for"Symptom-Syndrome Element"Based on Syndrome Differentiation Principle
目的 针对朱文锋教授创立的证素辨证学中各证素的"症状-证素"贡献值加和总积分不一致,诊断阈值却相同的问题,以冠心病为例,探究"症状-证素"诊断权值归一化前后对证素辨识的影响.方法 通过"古今医案云平台"收集整理中医治疗冠心病的案例,进行数据处理,通过对证素积分进行比例缩放的方法对证素积分进行归一化处理,分析归一化前后证素辨识准确率.结果 冠心病涉及胸闷、脉弦、气短等 207 种症状.涉及证素 38 种,占比超过 10%的证素有 13 种,主要病位证素为心、心神、肝、脾,主要病性证素为气虚、血瘀、阴虚、气滞.将 300 例冠心病医名医案的证素诊断结果按比例缩放法归一化"症状-证素"诊断权值后,证素:痰、心、血虚、心神诊断结果与原医案诊断一致性较原始证素积分辨证结果有所提高.结论 通过对证素诊断权值进行归一化,改善了不平衡证素的输出性能,提高了证素总积分较小的证素辨证准确性.
Objective To resolve the inconsistency between varying summed"symptom-pattern element"contribution scores and a fixed diagnostic threshold in syndrome element differentiation,this study explored the effect of normalizing"symptom-pattern element"diagnostic weights on pattern element identification before and after normalization,using coronary heart disease as the investigative model.Methods Clinical cases of coronary heart disease treated with traditional Chinese medicine were collected and processed via the"Ancient and Modern Medical Case Cloud Platform".Syndrome element scores were normalized through proportional scaling,and the accuracy of syndrome element identification was compared before and after normalization.Results Coronary heart disease involved 207 symptoms,including chest tightness,wiry pulse,shortness of breath,and others.A total of 38 syndrome elements were identified,among which 13 accounted for more than 10%of cases.The primary disease-location syndrome elements were the heart,heart spirit,liver,and spleen,while the main disease-nature syndrome elements included qi deficiency,blood stasis,yin deficiency,and qi stagnation.After normalizing the diagnostic weights of"symptom-syndrome element"relationships using the proportional scaling method based on 300 renowned medical case records of coronary heart disease,the diagnostic consistency of the syndrome elements phlegm,heart,blood deficiency,and heart spirit with the original medical diagnoses improved compared to the original syndrome element integral pattern differentiation results.Conclusion The normalization of diagnostic weights for syndrome elements enhance the output performance of imbalanced syndrome elements and improve the accuracy of pattern differentiation for syndrome elements with lower total integral values.
