多学科联合分级管理模式在提高住院患者抗菌药物治疗前病原学送检率中的作用OA
Role of multidisciplinary collaborative hierarchical management model in the improvement of pathogenic microorganism testing rate of inpatients before antimicrobial treatment
目的 评价多学科联合分级管理模式对提升二级甲等综合医院住院患者抗菌药物治疗前病原学送检率的干预效果,为优化抗菌药物合理使用管理策略提供依据.方法 选取某二级医院 2022 年 1-12 月接受抗菌药物治疗的 17 920 例住院患者为对照组(常规管理),2023 年 1-12 月实施多学科联合分级管理干预,选取 2024 年 1-12月接受抗菌药物治疗的 22 212 例住院患者为干预组,比较两组抗菌药物治疗前病原学送检率等核心指标差异.结果 干预后,抗菌药物治疗前病原学送检率从 56.18%提升至 68.80%(χ2=678.749,P<0.001),医院感染诊断相关病原学送检率从 62.96%提升至 85.39%(χ2=13.350,P<0.001),指向性、非指向性病原学送检率均显著升高(均 P<0.001);联合使用重点药物前病原学送检率(94.16%)与干预前比较差异无统计学意义(均 P>0.05).结论 多学科联合分级管理可有效提升住院患者抗菌药物治疗前病原学送检率,但部分指标仍未达国家规范要求,需进一步优化急危重症场景下的送检流程.
Objective To evaluate the intervention effect of a multidisciplinary collaborative hierarchical management model on improving the pathogenic microorganism testing rate before antimicrobial therapy among inpatients in a secondary grade A general hospital,so as to provide evidence for optimizing the management strategy of rational use of antimicrobial agents.Methods A total of 17 920 inpatients who received antimicrobial therapy from January to December 2022 in a sec-ondary hospital were selected as control group(conventional management).The multidisciplinary collaborative hierarchical management intervention was implemented from January to December 2023,and 22 212 inpatients who received antimicrobial therapy from January to December 2024 were selected as intervention group.Differences in core indicators such as the patho-genic microorganism testing rate before antimicrobial therapy were compared between the two groups.Results After inter-vention,the pathogenic microorganism testing rate before antimicrobial therapy increased from 56.18%to 68.80%(χ2=678.749,P<0.001),the testing rate of pathogenic microorganisms related to nosocomial infection diagnosis increased from 62.96%to 85.39%(χ2=13.350,P<0.001),and the testing rates of both targeted and non-targeted pathogens increased significantly(all P<0.001).The pathogen detection rate before the combined use of key drugs(94.16%)showed no statis-tically significant difference compared to that before the intervention(all P>0.05).Conclusion Multidisciplinary collabo-rative hierarchical management can effectively improve the pathogenic microorganism testing rate before antimicrobial therapy among inpatients,but some indicators still do not meet the national standard requirements,and it is necessary to further opti-mize the testing process in acute and critical care scenarios.
