Research progress in immunotargeted therapy for alopecia areata

斑秃(AA)是一种常见的非瘢痕性脱发,免疫异常和遗传易感性为其主要病因,但具体发病机制仍待阐明.近年来,随着对 AA 免疫发病机制(如毛囊免疫赦免丧失、JAK-STAT 信号通路激活、Th 细胞失衡等)的深入探索,免疫靶向治疗已成为研究热点.本文系统综述了 AA 免疫靶向治疗的研究进展,涵盖 JAK 抑制剂、Th 细胞相关靶点(IFN-γ、IL-17 等)、免疫检查点调节剂及新型靶点(如 S1P 受体、pDC 单抗)等方向,并探讨了现有药物的疗效与局限性,展望了精准靶向联合治疗的应用前景.

Alopecia areata(AA)is a prevalent non-scarring alopecia driven primarily by immune dysregulation and genetic pre-disposition,but its precise pathogenesis remains incompletely understood.In recent years,with the in-depth exploration of the immuno-pathogenesis of AA(such as the collapse of hair follicle immune privilege,activation of the JAK-STAT signaling pathway,and Th cell imbalance),immunotargeted therapy has emerged as a research hotspot.This review systematically summarizes current progress in im-munotargeted therapy for AA,including JAK inhibitors,Th-related cytokines(e.g.,IFN-γ,IL-17),immune-checkpoint modulators,and emerging targets such as sphingosine-1-phosphate(S1P)receptor modulators and plasmacytoid dendritic cell(pDC)-depleting monoclonal antibodies.The efficacy and limitations of existing agents are discussed,and the prospects of precision-targeted combination therapy are envisioned.
