急性缺血性脑卒中(AIS)后的血压管理是临床核心问题.血压异常波动可加重脑损伤、影响神经功能预后,现有基于人群的指南推荐无法充分体现患者脑血流调节能力的个体差异,难以制定精准血压控制目标.动态脑血流自动调节(dCA)是维持脑灌注稳态的关键生理机制,其动态评估为 AIS 个体化血压管理提供了全新方向.经颅多普勒超声(TCD)具备无创、实时、床旁可重复监测及高时间分辨率等优势,是评估 dCA 功能的重要手段.本文系统综述 TCD 评估dCA 的技术方法与生理学基础,重点阐述 TCD 相关参数在判别 AIS 后脑灌注调节能力、探寻个体化理想血压区间中的应用价值,总结其在 AIS 血压管理中的临床证据、转化困境与未来研究方向.基于TCD的dCA评估有望为AIS 患者个体化血压管理提供更符合病理生理特征的决策依据,但其常规临床应用仍需评估流程进一步标准化,并有待高质量临床研究加以验证.

Blood pressure management after acute ischemic stroke(AIS)is a core clinical issue.Abnormal blood pressure fluctuations can exacerbate brain injury and affect neurological functional outcomes.Current population-based guideline recommendations cannot fully capture the individual differences in patients'cerebral blood flow regulation capacity,making it difficult to establish precise blood pressure control targets.Dynamic cerebral autoregulation(dCA)plays a central role in maintaining cerebral perfusion homeostasis,and its dynamic assessment provides a new direction for personalized blood pressure management in AIS patients.Transcranial Doppler ultrasound(TCD)offers advantages such as being non-invasive,real-time,repeatable at the bedside,and having high temporal resolution,making it an important tool for assessing dCA function.This article systematically reviews the technical methods and physiological basis of using TCD to assess dCA,with a focus on the application value of TCD-related parameters in identifying cerebral perfusion regulation capacity after AIS and exploring individualized optimal blood pressure ranges.It also summarizes the clinical evidence,translational barriers,and future research directions in blood pressure management of AIS.TCD-based dCA assessment holds promise for providing a decision-making basis that better aligns with pathophysiological characteristics for individualized blood pressure management in AIS patients.However,its routine clinical application still requires further standardization of the assessment process and awaits validation through high-quality clinical studies.
