早产儿有血流动力学意义的动脉导管未闭的影响因素分析OA
Analysis of influencing factors of hemodynamically significant patent ductus arteriosus in premature infants
目的 分析早产儿有血流动力学意义的动脉导管未闭(hsPDA)的影响因素.方法 选取 2024 年1月至12 月南方医科大学深圳妇幼保健院新生儿科收治的150 例动脉导管未闭早产儿,根据心脏超声血流动力学标准结果分为hsPDA组(n=98)和未发生hsPDA组(n=52).通过单因素、logistic回归分析hsPDA的影响因素.结果 单因素分析显示,胎龄、出生体重、败血症、呼吸窘迫综合征(RDS)、血小板计数(PLT)、血小板压积(PCT)、大血小板比率(P-LCR)、平均血小板体积(MPV)及血小板分布宽度(PDW)与hsPDA相关(P<0.05);多因素logistic回归分析显示,RDS、低PLT、低PCT、低P-LCR、高MPV、高PDW是hsPDA的独立危险因素,出生体重为保护因素(P<0.05).结论 早产儿hsPDA的发生与血小板功能低下及RDS密切相关,监测早期血小板参数可辅助临床干预.
Objective To analyze the influencing factors of hemodynamically significant patent ductus arteriosus(hsPDA)in premature infants.Methods A total of 150 premature infants with patent ductus arteriosus admitted to Department of Neonatology,Shenzhen Maternity&Child Healthcare Hospital,Southern Medical University from January to December 2024 were selected.According to the results of cardiac ultrasound hemodynamic standards,they were divided into hsPDA group(n=98)and non-hsPDA group(n=52).The single factor and logistic regression were used to analyze the influencing factors of hsPDA.Results The single-factor analysis showed that gestational age,birth weight,sepsis,respiratory distress syndrome(RDS),platelet count(PLT),platelet distribution width(PCT),large platelet ratio(P-LCR),mean platelet volume(MPV),and platelet distribution width(PDW)were associated with hsPDA(P<0.05).The multivariate logistic regression analysis revealed that RDS,low PLT,low PCT,low P-LCR,high MPV,and high PDW were independent risk factors for hsPDA,while birth weight was a protective factor(P<0.05).Conclusion The occurrence of hsPDA in premature infants is closely related to low platelet function and RDS.Monitoring of early platelet parameters can assist in clinical intervention.
