特发性肺纤维化与胃食管反流病相互影响的中西医研究进展OA
Research Progress on the Interaction between Idiopathic Pulmonary Fibrosis and Gastroesophageal Reflux Disease in Traditional Chinese and Western Medicine
特发性肺纤维化(idiopathic pulmonary fibrosis,IPF)是一个病因不明的慢性进行性纤维化间质性肺炎,患者预后差,病死率高,目前对其病理机制的研究尚不清楚.治疗IPF的药物目前只有吡非尼酮和尼达尼布,但治疗效果稍差,不能彻底治愈.胃食管反流病(gastroesophageal reflux disease,GERD)在IPF患者中患病率高,经研究发现GRED相关的微呼吸导致肺部结构的持续性炎症,从而加速IPF的进展,可促进或诱导肺纤维化,而肺弹性差引起的呼吸力学变化也可能增加反流,两者相互影响.中医认为,IPF与GERD发病多为肺脾虚,肺失宣发、肃降作用,脾失运化,两者相互影响,进而痰、瘀、毒互结于肺.文章探讨GERD与IPF的长期相互影响,从而为IPF的治疗提供新的思路与看法.
Idiopathic pulmonary fibrosis(IPF)is a chronic progressive interstitial pneumonia with unknown etiology.The prognosis of patients is poor and the mortality is high.At present,the research on its pathological mechanism is still unclear.At present,only pirfenidone and Nidanib are used to treat IPF,but the treatment effect is slightly poor,which can not completely cure patients.The prevalence of gastroesophageal reflux disease(GERD)is high in IPF patients.It is found that micro-breathing re-lated to GERD leads to persistent inflammation of lung structure,which accelerates the progress of IPF and can promote or induce pulmonary fibrosis.The respiratory mechanics changes caused by poor lung elasticity may also increase reflux,and the two may in-teract with each other.Traditional Chinese medicine believes that IPF and GERD are mostly caused by lung spleen deficiency,lung failure to declare and relieve depression,and spleen failure to transport,and the two affect each other,and then phlegm,blood stasis and toxin are combined in the lung.This paper discusses the long-term interaction between GERD and IPF,so as to pro-vide new ideas and views for the treatment of IPF.
