糖尿病肾病(diabetic kidney disease,DKD)作为糖尿病最严重的微血管并发症之一,在管理欠佳的情况下会进展为终末期肾病.炎症是 DKD 早期的驱动因素,更是推向终末期肾病的关键机制,抗炎治疗贯穿于 DKD始终.近年来关于转化生长因子-β(Transforming growth factor-β,TGF-β)/Smad 信号通路在 DKD 中的作用备受关注,DKD 高糖状态导致 TGF-β 信号异常激活,引发级联炎症反应、细胞外基质沉积进而推动肾纤维化进程,该信号通路是参与炎症反应和纤维化的关键通路,是引起肾损伤的核心推动力.通过系统综述 TGF-β/Smad 信号通路在DKD 炎症反应中的作用机制,总结中药单药和复方及其有效成分通过调控该信号通路治疗 DKD 的研究进展,为DKD 的中医药干预和治疗提供新的思路和理论依据.

Diabetic nephropathy(DKD),one of the most severe microvascular complications of diabetes,can progress to end-stage renal disease if poorly managed,leading to the need for kidney replacement therapy,which poses significant physical harm and economic burden to patients.Inflammation is a key driver in the early stages of DKD and a critical mechanism leading to end-stage renal disease.Anti-inflammatory treatment is essential throughout the course of DKD.In recent years,the role of the transforming growth factor-β(TGF-β)/Smad signaling pathway in DKD has gained significant attention.The hyperglycemic state in DKD leads to abnormal activation of the TGF-β signaling pathway,triggering a cascade of inflammatory responses,extracellular matrix deposition,and promoting renal fibrosis.This signaling pathway is a key player in inflammation and fibrosis,driving renal damage.This article systematically reviews the mechanisms by which the TGF-β/Smad signaling pathway plays a role in the inflammatory response of DKD,and explores the research progress of TCM monomers and combinations,as well as their active ingredients,in treating DKD by regulating this signaling pathway,providing new ideas and theoretical foundations for TCM intervention and treatment of DKD.
