目的:探讨壮医祛毒通养方对多囊卵巢综合征胰岛素抵抗(PCOS-IR)大鼠的作用机制.方法:将60只斯泼累格·多雷(SD)雌性大鼠按随机数字法分为空白对照组、模型对照组、阳性对照组及壮医祛毒通养方低、中、高剂量组,每组10只.除空白对照组外均用高脂饮食联合来曲唑灌胃构建PCOS-IR模型.干预21 d后,观察动情周期,检测空腹血糖(FBG)、空腹胰岛素(FINS)、促黄体素(LH)、促卵泡素(FSH)、睾酮(T)及雌二醇(E2)水平、胰岛素抵抗指数(HOMA-IR)及卵巢组织磷脂酰肌醇3-激酶(PI3K)/蛋白激酶B(AKT)/葡萄糖转运蛋白4(GLUT4)信号通路相关分子表达,评估卵巢脏器系数及病理形态.结果:与空白对照组比较,模型对照组大鼠动情周期紊乱,卵巢呈多囊样改变,颗粒细胞层数减少;FBG、FINS、HOMA-IR、LH、T 水平及卵巢系数高(P<0.05);E2、FSH 水平及相关 mRNA(PI3K、AKT、GLUT4)和蛋白(PI3K、AKT、p-AKT、GLUT4)表达低(P<0.05).与模型对照组比较,壮医祛毒通养方各剂量组卵巢病理改善,囊状卵泡减少,颗粒细胞层增厚;FBG、FINS、HOMA-IR、LH、T及卵巢系显著下降(P<0.05);E2、FSH及通路上述分子表达显著提高(P<0.05).结论:壮医祛毒通养方可改善PCOS大鼠改善胰岛素抵抗及卵巢多囊样病变等,机制可能与激活PI3K/AKT/GLUT4信号通路相关.

Objective:To investigate the mechanisms of the Zhuang medicine Qudu Tongyang Formula in rats with polycystic ovary syndrome and insulin resistance(PCOS-IR).Methods:Sixty female Sprague Dawley(SD)rats were randomly divided into a blank control group,a model control group,a positive control group,and low-,medium-,and high-dose Qudu Tongyang Formula groups,with 10 rats in each group.Except for the blank control group,all other groups were subjected to a high-fat diet combined with letro-zole gavage to establish the PCOS-IR model.After 21 days of intervention,estrous cycle changes were observed.Fasting blood glu-cose(FBG),fasting insulin(FINS),luteinizing hormone(LH),follicle-stimulating hormone(FSH),testosterone(T),and estradiol(E2)levels were measured,and the insulin resistance index(HOMA-IR)was calculated.The expression levels of phosphatidyli-nositol 3-kinase(PI3K)/protein kinase B(AKT)/glucose transporter 4(GLUT4)signaling pathway-related molecules in ovarian tissue were detected.Ovarian organ coefficients and pathological morphology were evaluated.Results:Compared with the blank con-trol group,the model control group showed estrous cycle disorder,polycystic ovarian changes,and reduced granulosa cell layers.FBG,FINS,HOMA-IR,LH,T levels,and ovarian coefficients were significantly increased(P<0.05),while E2 and FSH levels and the mRNA expression of PI3K,AKT,GLUT4,as well as the protein expression of PI3K,AKT,p-AKT,and GLUT4,were significant-ly decreased(P<0.05).Compared with the model control group,all dose groups of the Qudu Tongyang Formula showed improve-ment in ovarian pathology,with reduced cystic follicles and thickening of the granulosa cell layer.FBG,FINS,HOMA-IR,LH,T lev-els,and ovarian coefficients were significantly decreased(P<0.05),while E2,FSH levels,and the above pathway-related mRNA and protein expression levels were significantly increased(P<0.05).Conclusion:The Zhuang medicine Qudu Tongyang Formula can improve insulin resistance and alleviate polycystic ovarian changes in PCOS rats.Its mechanism may be related to activation of the PI3K/AKT/GLUT4 signaling pathway.
