Effect of Nebulized Inhalation on Clinical Outcomes in Patients with COPD Complicated by Sleep Disorders

目的:探讨在常规治疗基础上,联合雾化吸入对慢性阻塞性肺疾病(COPD)急性加重期合并睡眠障碍患者临床结局的影响.方法:选取2023年1月至2025年1月杭州市第一人民医院桐庐医院收治的COPD急性加重期,且匹兹堡睡眠质量指数(PSQI)>7分的患者80例作为研究对象,随机分为对照组和观察组,每组40例.对照组接受常规综合治疗,观察组在此基础上每天加用2次布地奈德、特布他林与异丙托溴铵雾化吸入.比较2组治疗前后PSQI、睡眠状况自评量表(SRSS)、改良英国医学研究委员会(mMRC)呼吸困难、COPD评估测试(CAT)、夜间症状视觉模拟量表(VAS)评分及肺功能指标.结果:治疗后,观察组PSQI(8.15±2.12对比10.82±2.36)、SRSS(22.74±3.85对比26.87±4.02)、mMRC(1.85±0.61 对比 2.31±0.68)、CAT(17.34±3.85 对比 20.72±4.12)及夜间 VAS 评分(4.12±1.05 对比 5.87±1.18)均显著低于对照组(均P<0.05);观察组FEV1及FEV1/FVC改善差异有统计学意义(P<0.05);2组不良反应发生率差异无统计学意义(P>0.05).结论:对伴有睡眠障碍的AECOPD患者,在常规治疗基础上联合短期强化雾化吸入,可进一步改善睡眠、症状、肺功能与生命质量,且安全性良好.

Objective:To explore the effect of combined nebulized inhalation therapy on clinical outcomes in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated by sleep disorders,based on routine treatment.Methods:A total of 80 patients with AECOPD and Pittsburgh Sleep Quality Index(PSQI)scores>7 admitted to Tonglu Hospital of Hangzhou First People's Hospital from January 2023 to January 2025 were enrolled and randomly divided into a con-trol group and an observation group,with 40 cases in each group.The control group received routine comprehensive treatment,while the observation group additionally received nebulized inhalation of budesonide,terbutaline,and ipratropium bromide twice daily.PSQI,Self-Rating Scale of Sleep(SRSS),modified Medical Research Council(mMRC)dyspnea scale,COPD Assessment Test(CAT),nighttime symptom Visual Analogue Scale(VAS),and pulmonary function indices were compared before and after treatment.Results:After treatment,the observation group showed significantly lower scores in PSQI(8.15±2.12 vs 10.82±2.36),SRSS(22.74±3.85 vs 26.87±4.02),mMRC(1.85±0.61 vs 2.31±0.68),CAT(17.34±3.85 vs 20.72±4.12),and nighttime VAS(4.12±1.05 vs 5.87±1.18)compared with the control group(all P<0.05).Improvements in FEV1 and FEV1/FVC were also more significant in the observation group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:For patients with AECOPD complicated by sleep disorders,short-term intensified nebulized inhalation therapy in addition to routine treatment can further improve sleep quality,symptoms,pulmonary function,and quality of life,with good safety.
