Effect of electroacupuncture combined with Western medication on intestinal function in patients with constipation after intestinal tumor surgery

目的 基于脑-肠轴探讨电针对肠道肿瘤术后便秘患者肠道功能及临床症状的影响.方法 将 90 例肠道肿瘤术后便秘患者随机分为观察组(45 例;剔除1例,脱落3 例)和对照组(45例;剔除1 例,脱落4 例).对照组给予西医常规治疗,观察组在对照组基础上加用电针治疗.比较两组临床疗效、肠道功能(排便频率、排便时间、粪便性状及肠道传输时间)、临床症状(便秘症状评分及腹痛、腹胀程度)、脑-肠轴相关指标[血清 P 物质(substance P,SP)、血管活性肠肽(vasoactive intestinal peptide,VIP)、5-羟色胺(5-hydroxytryptamine,5-HT)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)和白细胞介素-6(interleukin-6,IL-6)水平]、粪便肠道菌群[厚壁菌门(Firmicutes)、拟杆菌门(Bacteroidetes)及其比值(F/B),以及双歧杆菌(Bifidobacterium)、乳酸杆菌(Lactobacillus)、大肠杆菌(Escherichia)菌属的相对丰度]指标.结果 治疗后,观察组排便频率、粪便性状显著高于对照组(P<0.05),排便时间及肠道传输时间显著低于对照组(P<0.05);观察组便秘症状评分、VAS评分显著低于对照组(P<0.05);观察组血清SP、VIP、5-HT显著高于对照组(P<0.05);观察组血清 IL-6、TNF-α显著低于对照组(P<0.05);治疗后,观察组粪便肠道菌 Chao1 指数、Shannon 指数显著高于对照组(P<0.05),F/B 比值显著低于对照组(P<0.05);观察组粪便 Firmicutes、Escherichia显著低于对照组(P<0.05),Bacteroidetes、Bifidobacterium、Lactobacillus显著高于对照组(P<0.05).观察组总有效率为92.7%,显著高于对照组的77.5%(P<0.05).结论 电针联合西医常规治疗可促进肠道肿瘤术后便秘患者血清 SP、VIP、5-HT释放,加速肠道传输;抑制血清 IL-6、TNF-α释放,减轻肠道炎症反应;还能优化肠道菌群结构,继而从多靶点恢复肠道功能.

Objective To observe the effects of electroacupuncture(EA)on intestinal function and clinical symptoms in patients with constipation after intestinal tumor surgery based on the brain-gut axis.Method Ninety patients with constipation after intestinal tumor surgery were randomly assigned to an observation group(45 cases;1 case of elimination and 3 dropouts)and a control group(45 cases;1 case of elimination and 4 dropouts).The control group received conventional Western medicine treatment,and the observation group additionally received EA treatment.The two groups were compared in terms of therapeutic efficacy,intestinal function(bowel movement frequency,defecation time,stool consistency,and intestinal transit time),clinical symptoms(constipation symptom scores and abdominal pain and bloating intensities),brain-gut axis-associated indicators[the serum levels of substance P(SP),vasoactive intestinal peptide(VIP),5-hydroxytrptamine(5-HT),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)],and gut microbiota in fecal samples[Firmicutes and Bacteroidetes and their ratios(F/B),as well as the relative abundance of Bifidobacterium,Lactobacillus,Escherichia].Result After treatment,the observation group had higher bowel movement frequencies and stool consistency scores compared to the control group(P<0.05),and shorter defecation time and intestinal transit time(P<0.05);the constipation symptom and visual analog scale(VAS)scores were significantly lower in the observation group than in the control group(P<0.05);the levels of serum SP,VIP,and 5-HT were significantly higher in the observation group than in the control group(P<0.05);the observation group had markedly lower levels of serum IL-6 and TNF-α compared to the control group(P<0.05);compared to the control group,the observation group exhibited notably higher Chao1 and Shannon indexes of gut microbiota in fecal samples(P<0.05)and a significantly lower F/B(P<0.05);compared to the control group,the observation group demonstrated significantly lower levels of Firmicutes and Escherichia(P<0.05)and higher levels of Bacteroidetes,Bifidobacterium,and Lactobacillus(P<0.05).The observation group achieved a total effective rate of 92.7%,significantly higher than 77.5%in the control group(P<0.05).Conclusion The combined use of EA and conventional Western medicine treatment can promote the release of serum SP,VIP,and 5-HT,to accelerate intestinal transit,inhibit inflammatory factors serum IL-6 and TNF-α to reduce inflammatory reactions,and optimize the composition of gut microbiota in patients with constipation after intestinal tumor surgery,thereby recovering intestinal function through multiple targets.
