目的 观察电针联合益生菌治疗重度颅脑损伤(severe traumatic brain injury,sTBI)合并胃肠功能障碍的临床疗效.方法 将 sTBI 合并胃肠功能障碍患者90 例,采用随机数字法分为电针组、益生菌组和联合组,每组 30 例.电针组、益生菌组和联合组在加速康复外科基础上于术后当日分别给予电针、口服益生菌和电针联合口服益生菌治疗.观察 3 组治疗前后改良术后胃肠功能紊乱(postoperative gastrointestinal dysfunction,PGD)、中医证候积分、胃肠道功能障碍评分(gastrointestinal dysfunction score,GIDS)、肠功能生物标记物[血清促胃液素(gastrin,GAS)和促胃动素(motilin,MOT)]水平、急性胃肠损伤生物标记物[血清内毒素(endotoxin,ET)、二胺氧化酶(diamine oxidase,DAO)和 D-乳酸(lactate,LAC)]水平变化,比较3 组肠鸣音恢复时间、排气时间和排便时间,并比较3组临床疗效及不良反应情况.结果 治疗后,3 组中医证候积分和GIDS评分均低于治疗前;组间比较差异有统计学意义(P<0.05),联合组显著低于其余2组(P<0.05).治疗后,3组肠鸣音恢复时间、肠道排气时间和肠道排便时间比较差异有统计学意义(P<0.05),联合组短于电针组和益生菌组(P<0.05).治疗后,3 组血清 GAS 和 MOT 水平高于治疗前(P<0.05);组间比较差异有统计学意义(P<0.05),联合组高于其余 2 组(P<0.05).治疗后,3 组血清 ET、DAO 和 LAC 水平低于治疗前(P<0.05);组间比较差异有统计学意义(P<0.05),且联合组低于其余 2 组(P<0.05).3 组临床疗效比较,差异有统计学意义(P<0.05);联合组优于其余2 组(P<0.05).3组均未出现导致退出治疗的严重不良反应.结论 电针联合益生菌对改善 sTBI 患者胃肠功能疗效明确,优于单一电针或益生菌治疗.

Objective To observe the clinical efficacy of electroacupuncture combined with probiotics in the treatment of severe traumatic brain injury(sTBI)complicated with gastrointestinal dysfunction.Method Ninety patients with sTBI complicated with gastrointestinal dysfunction were enrolled and randomly divided,using a random number table,into an electroacupuncture group,a probiotic group,and a combination group,with 30 cases in each group.On the basis of enhanced recovery after surgery(ERAS),patients in the electroacupuncture group received electroacupuncture,those in the probiotic group received oral probiotics,and those in the combination group received electroacupuncture combined with oral probiotics,starting on the day of surgery.Changes before and after treatment in modified postoperative gastrointestinal dysfunction(PGD)traditional Chinese medicine(TCM)syndrome scores,gastrointestinal dysfunction scores(GIDS),gastrointestinal function biomarkers[serum gastrin(GAS)and motilin(MOT)],and biomarkers of acute gastrointestinal injury[serum endotoxin(ET),diamine oxidase(DAO),and D-lactate(LAC)]were observed.Recovery time of bowel sounds,time to first flatus,and time to first defecation were compared among the three groups.Clinical efficacy and adverse reactions were also evaluated.Result After treatment,TCM syndrome scores and GIDS scores were significantly lower than before treatment in all three groups.There were statistically significant differences among groups(P<0.05),with the combination group showing significantly lower scores than the other two groups(P<0.05).Significant differences were observed among the three groups in recovery time of bowel sounds,time to first flatus,and time to first defecation after treatment(P<0.05),all of which were shorter in the combination group than in the electroacupuncture group and the probiotic group(P<0.05).After treatment,serum levels of GAS and MOT were significantly higher than before treatment in all three groups(P<0.05);intergroup comparisons showed statistically significant differences(P<0.05),with the combination group having higher levels than the other two groups(P<0.05).Serum levels of ET,DAO,and LAC were significantly lower after treatment in all three groups(P<0.05),with the combination group showing significantly lower levels than the other two groups(P<0.05).There were statistically significant differences in overall clinical efficacy among the three groups(P<0.05),and the combination group demonstrated superior efficacy compared with the other two groups(P<0.05).No serious adverse reactions leading to treatment withdrawal occurred in any group.Conclusion Electroacupuncture combined with probiotics is effective in improving gastrointestinal function in patients with severe traumatic brain injury and is superior to electroacupuncture or probiotic therapy alone.
